Seven Hills Foundation is Hiring an EI Service Coordinator 2 Near Worcester, MA

Position Summary:Works with and supports family members and other caregivers to enhance the development of infants and toddlers with developmental delays/disabilities through daily routines and learning opportunities. Functions as a member of a multidisciplinary team using a coaching approach. Performs service coordination, evaluation and assessment, IFSP development and maintenance, and delivery of EI services requiring a “Level II Practitioner”. Monitors and assists the child and family in programmatic interventions including but not limited to gaining access to needed medical, social, educational and other developmental/therapeutic services.Qualifications:Master’s Degree and/or certification/licensure in Early Childhood Education or Special Education, Child Development, Social Work, Nursing, Psychology, Nutrition, or related Early Intervention field. 2-5 years’ experience working with infants/toddlers and their families in a family focused, homebased setting; experience in transdisciplinary provision of support. A minimum of 12 hours per year professional development with focus on working with young children with disabilities and their families. Completion of Introduction to EI within 6 months of date of employment required. Certification in Routines Based Interviewing within 12 months of employment required. Must have valid driver’s license, safe driving record, automobile insurance and own vehicle to be used as directed and when needed for transportation. Must have and maintain pediatric CPR certification.Essential Functions:Protects the rights and dignities of individuals with developmental disabilities and extend these rights and dignities to family members or guardians.Upholds and complies with all safety programs and policies to maintain a safe work environment for employees, clients, family members and visitors.Provides support and education to families and other caregivers to incorporate IFSP outcomes into the daily routines of each infant/toddler through a coaching approach.Conducts intakes and participates in the eligibility evaluation/assessment process.Utilizes Routines Based Interviewing to develop understanding of each family’s strengths, needs and priorities as basis for writing goals/outcomes.Participates in the development of goals, planning and assessment of progress in meeting the goals and objectives of the IFSP.Coordinates and chairs as appropriate team meetings including IFSP, IFSP reviews and Transition Conference.Facilitates the development and implementation of transition planning for each child exiting EI in accordance with state and federal guidelines.Assists families in gaining access to EI and other services listed in the IFSP; facilitates, coordinates, and monitors delivery of services and assists family in identifying resources and available service providers and making referrals as needed.Commits to organizational quality initiatives by participating in programs that will assure quality improvements and team processes.Documents all activities in accordance with requirements necessary for fulfilling, agency policies or Early Intervention regulations.Obtains consultation from other members of the team in areas outside own expertise in order to fully address the needs of individual children on case by case basis.Maintains professional knowledge through participation in professional organizations, conferences and in-services.Provides direct and/or indirect Early Intervention services as qualified.Performs evaluations and/or assessments in their specific discipline.Mentors Service Coordinator I(s) and interns.Provides formal teaching to parents and other members of the team in area of expertise.
