Job Overview:
We are seeking a dedicated and compassionate Guardianship Case Manager to join our team. Start your career as a court-appointed Guardian with this hybrid, full-time position. You will enjoy on-the-job training and employer-sponsored support toward professional certification and future advancement. You will help older adults make important decisions , serving those who have cognitive impairments. Knowledge of long term care, Social Security benefits, case management and medical terminology are desirable.
Independent self-starters interested in this position must have valid PA driver's license, proof of car insurance, and ability to travel throughout Northeast Pennsylvania. As a largely on-the-road, remote employee, you should be comfortable with MS Office suite and web-based platforms.
Duties:
- Conduct assessments of clients' needs, strengths, and goals
- Develop individualized care plans in collaboration with clients and their support systems
- Coordinate and monitor the delivery of services to ensure clients' needs are met
- Collaborate with other professionals, such as social workers, healthcare providers, and educators, to ensure holistic care for clients
- Advocate for clients' rights and access to resources
- Maintain accurate and up-to-date client records and documentation
- Stay informed about current Guardianship best practices
Requirements:
- Bachelor's degree, but an Associates considered with relevant experience
- Experience as a case manager or in serving older adults a plus
- A clean criminal background check
- PA Drivers License and auto insurance
- Crisis intervention and conflict management skills
- Excellent communication and interpersonal skills
- Ability to work collaboratively with diverse individuals and teams
- Strong organizational skills with the ability to manage multiple cases simultaneously
- Proficiency in maintaining accurate documentation and records
- Willingness to pursue and obtain Certified National Guardian status
