Summary of Responsibilities:

Under the general direction of the Blended Case Management Supervisor, this person provides Blended Case Management for persons served. This person is proficient in the use of a personal computer and is adept at handling multiple tasks at one time. This person has expertise in providing good customer service. This person is expected to exercise initiative and judgment in discharging responsibilities. Duties are conducted in accordance with all existing policies, procedures, and regulations and in accord with the Mission and Values of the Company.

Essential Functions:


  • Builds relationships

  • Leads teams to support the people we serve

  • Collaborates with individuals, families, and providers to ensure people are living their best lives

  • Facilitates meetings and discussions

  • Advocates

  • Thinks critically and problem solves

  • Provides crisis intervention

  • Links individuals to community resources

  • Assesses needs

  • Completes and updates Individualized Service Plans

  • Completes case notes, & Available to work on-call shifts.

Education and Experience:

Bachelor's Degree in psychology, social work, counseling, social welfare, sociology, vocational rehabilitation, or related field. Or, 12 credit hours in psychology, social work, counseling, social welfare, sociology, vocational rehabilitation, or a related field AND 2 years experience.

MH/DD experience preferred

Employment Requirements:

Automobile and Valid Insurance, Valid Pennsylvania Driver's License.

Working Conditions:

Prolonged periods working on a computer in an open office environment, comfortable communicating through virtual technology, work may be required at nights and on weekends, travel throughout the region, travel may be in adverse weather conditions, must be able to lift to 15 pounds.
