Mental Health Blended Case Manager
Position Summary:
Mental Health Case Managers work with individuals in the community to overcome their barriers and obstacles that confront them to recover from their mental illness. They provide a service that includes functions that involve locating, coordinating, and monitoring services and supports for individuals who have a mental illness.
Day Hours, M-F with on-call (standby rate/overtime, rotating basis).

Qualifications:
Bachelor’s degree, which includes or is supplemented by at least 12 college credits in sociology, social work, psychology, gerontology, criminal justice, or other related social science; OR
Associate degree which includes or is supplemented by at least 12 college credits in sociology, social work, psychology, gerontology, criminal justice or other related social science AND two years of relevant experience
Automobile, Valid Driver’s license, PA Child Abuse, PA Criminal History and FBI clearances are required
