Intake Support Specialist

Position Overview:

The Intake Support Specialist receives all initial inquiry calls about Sequoia Mental Health Services. The Intake Support Specialist provides information about agency services. Determines appropriateness of service for the client referred, and refers inappropriate clients to other agencies. The Intake Support Specialist provides information as well as referrals on immediate needs: housing, shelters, medical care, dental care and food assistance as needed. The Intake Support Specialist prepares clinical documents for enrollment. Knowledge of behavioral health terminology and HIPPA preferred.

Essential Job Functions:

  • Receive and return incoming calls for services.
  • Screen calls for eligibility including insurance and income verifications.
  • Coordinate and consult with referral agencies, hospital social workers, parole officers, jail liaisons and the crisis team. Schedule interviews at hospitals, with the crisis team, at the jail or probation department, or other locations as needed. Get discharge summaries as needed.
  • Follow up calls and reminders to referred clients. Document outreach efforts.
  • Complete screening form. Establish eligibility for service. Get insurance information. Assist in transfer to Washington County if needed.
  • Schedule intake appointments. Complete all paperwork as detailed on Intake Checklist. Complete General Fund eligibility or OHP authorization.
  • Explain to client and/or guardian agency privacy practices, grievance procedure and mental health advance directive, and obtain written acknowledgment of receipt and understanding.
  • Obtain consent from Guardian when necessary.
  • Request records from all completed Releases of Information (ROI).
  • Prepare and submit monthly ACCESS reports as required.

Required Skills:

  • Be highly organized with a positive demeanor and good people skills.
  • Knowledgeable about agency and community services.
  • Proficient in computer application including Microsoft office suite and electronic medical records software.

  • Ability to gather histories of mental and physical health, alcohol and other drug use, past mental health services and criminal justice contacts, family information, as well as cultural social and work relationships.

Physical Requirements:

  • Ability to meet the following physical requirements with or without reasonable accommodation:
  • Use hands to manipulate, handle, feel, and control items or equipment;
  • Walk, bend, kneel, and reach;
  • Lift a 20-60 lb.;
  • Stand and/or sit for long periods of time;
  • Talk, hear, and communicate with clients;
  • See and be able to read, write, and interpret written documents.

Education:

  • Meeting Oregon requirements as a Qualified Mental Health Associate is a plus. Associate’s or Bachelor’s degree preferred.
