Sentara Community Care is expanding its services to include Maternal Health. We are seeking a NP/PA/CNM to support prenatal and infant health in our underserved and under-resourced communities.Highlights of this position include:
*Location: Richmond, VA
*These clinics will provide primary, prenatal, and postnatal care in an outpatient setting with the exclusion of delivery.
*Treat family medicine, pediatrics, and adults with complex issues
*Full-time position with non-traditional hours of noon to 6:00pm or 7:00pm, plus
1-2 Saturdays per month
*Shared call schedule, with RN first call
*Co-manage patients with a diverse care team to include, but not limited to, RN, Counselors, Care Management, and other team members to improve our patients' health holistically.
*Review quality and outcome metrics monthly with the healthcare team
*Collaborate with local civic and faith-based leaders on community education programs
*Experience working in a FQHC or Community Health Center, or post-masters training in Community Health Care would be of great benefit to the practice
