Sentara Community Care is expanding its services to include Maternal Health to support prenatal and infant health in our underserved communities. We are looking for a seasoned Family Nurse Practitioner or PA to care for Pediatrics and Adult Care, who is also comfortable providing some basic prenatal care. Some highlights of this position include:•Location: Richmond, VA•These clinics will provide primary, prenatal, and postnatal care in an outpatient setting with the exclusion of delivery.•Treat family medicine, pediatrics, and adults with complex issues•Full-time position with non-traditional hours of noon to 6:00pm or 7:00pm, plus 1-2 Saturdays per month •Shared call schedule, with RN first call•Co-manage patients with a diverse care team to include, but not limited to, RN, Counselors, Care Management, and other team members to improve our patients’ health holistically. •Review quality and outcome metrics monthly with the healthcare team•Collaborate with local civic and faith-based leaders on community education programs•Experience working in a FQHC or Community Health Center, or post-masters training in Community Health Care would be of great benefit to the practiceSentara Healthcare is an award-winning, nationally recognized, innovative nonprofit healthcare system, recently featured as one of America's Top 100 Employers.
