About Us

Reporting to the Manager, Care Management, the Supervisor, Care Management will supervise the day-to-day operations of a team of Care managers (RN and non-RN) whom provide case management services across the continuum of care (assessment, care planning, care coordination, and monitoring and evaluating services). Under the direction of the Manager, Care Management this individual will assist with creating action plans in support of quality improvement efforts and positive health outcomes, with a focus on building person-centered plans that fosters continuous improvement and caregiver engagement.

What You Will Do:

  • Supervise a team clinical and non-clinical staff to engage, motivate, and educate family caregivers to support complex individuals caring for their loved ones at home
  • Support compliance with clinical policy and best practices, including regulatory and contractual agreements as well as NCQA accreditation requirements
  • In collaboration with the Manager, Care Management, coach and develop staff to meet department goals, ensuring key performance indicators and goals are reviewed regularly
  • Assist Manager, Care Management with incident investigations and reporting
  • Oversee timely completion of care team activities, including assessments, care plans, and discharge planning
  • Provide oversight for the appropriate use of tools and person-centered care plans to support caregiver and care recipient needs; provide regular review of incidents to identify cause and recommend interventions
  • Report on key quality metrics including hospitalization rates, falls, incidents, activation, suspension, and discharge rates; identify improvement opportunities and manage interventions
  • Perform other duties and special projects as assigned

What You Will Bring:

  • 4 years of case management experience in healthcare and social services, preferably with experience in eldercare, and/or people with disabilities or other complex medical needs; 1 years of program or clinical operations management
  • 2 years of experience coaching or developing staff
  • Additional state-specific qualifications may be required
  • Ability to travel (usually by automobile) within assigned geographic area; valid driver’s license and automobile insurance is required
  • Working knowledge of case management systems, community service and delivery systems, and proficiency with Microsoft Office suite.
