Full Time

Position available for RN Case Manager who will float between Home Care and Williams Center programs providing nursing support, completion of participant assessments, and development and review of care plans. Position assesses and follows-up on the needs of the participant/family, makes referrals, provides education and information. RN case manager also assists in providing staff education and supports coaching and mentoring of aides and health care associates

Education, Experience, Training:

RN license in good standing with NC Board of Nursing. Current CPR and First Aid training must be maintained. Minimum of one year home health or home care experience and supervisory experience preferred. Experience working with older adults and people living with memory loss preferred.

Knowledge, Skills, Ability:

· Strong interpersonal skills and ability to build relationship and rapport quickly with a diverse array of people

· Excellent written and oral communications skills

· Attention to detail, accuracy, and strong documentation skills

· Solid organizational skills and ability to prioritize and navigate multiple tasks

· Flexible work style and ability to float between in-home care and center-care as needed

· Good computer skills, comfort learning new technology and processes

· Ability to handle sensitive information in a confidential manner
