As a Lead Care Manager, you will act as the primary contact person for youth and families requiring care coordination and community linkages. Lead Care Managers play a pivotal role in providing comprehensive care management and support serving as the cornerstone of the care coordination process and the driving force behind ensuring the holistic well-being of high-need youth and families.
ABOUT THE PROGRAM
The ECM Program is a Medi-Cal managed care benefit that addresses clinical and non-clinical needs of high-need youth and families through the coordination of services, connection to community resources, and comprehensive care management.
