The incumbent in this position is responsible for providing quality care to STOF’s patients through implementing and evaluating individual patient care plans, discharge planning and complex outpatient needs. The individual performs prospective, concurrent and retrospective evaluation of clinical information to establish medical necessity, appropriateness of services and cost effectiveness and serves as patient’s and family’s advocate during hospital stay and outpatient status. The incumbent in this position will also be required in some circumstances to complete an in-home or at hospital evaluation of patient, if deemed appropriate. Associate’s Degree or higher in Nursing is required. Current and valid State of Florida’s Registered Nurse License is required. Certification in Case Management is required, or has the ability to obtain within one year of employment. A minimum of two (2) years of experience working as a Medical Case Manager is required. Certification as Basic Life Support (BLS) and valid Florida Driver’s License are required. Must demonstrate extensive and varied background in the health care field with knowledge of current medical standards of practice is required. Must demonstrate excellent organizational and interpersonal skills. Must demonstrate proficiency utilizing Microsoft software packages. Must have the ability to work a flexible schedule including evenings, weekends and holidays.
