Self-Help For The Elderly is Hiring a Peer Ambassador Near San Francisco, CA

Title:Peer Ambassador

Department: Social Services

FLSA Status: Non-Exempt

Reports To: Program Supervisor

Summary: Conducts outreach activities and disseminate information about Department of Aging and Adult Services (DAS)-funded programs and services to organizations and individuals, including non-profits, faith-based organizations, businesses, health care providers, housing site coordinators and individuals who have contact with older adults and/or adults with disabilities living in the community.


Essential Functions:

  • Attends required meetings and training workshops.
  • Educates and informs seniors and adults with disabilities about DAS-funded programs and services available throughout the City and County of San Francisco.
  • Conducts outreach activities in the targeted outer districts in the City to promote community awareness of the DAS programs and services.
  • Participates in regularly scheduled meetings with DAS and Self-Help for the Elderly where there will be a coordinated exchange of program and service information.
  • Informs seniors and adults with disabilities about the DAS Benefits and Resources Hub streamlines access to the services available and makes referrals to the hub.
  • Helps with the distribution and collection of surveys at outreach activities to obtain feedback that will be used as a program evaluation tool.
  • Assists with the collection of data that will be entered into the DAS database.
  • Documents services according to program protocol and submits monthly service logs and reports.
  • Establishes and maintains good working relationships with staff at outreach activity sites.
  • Performs other duties as assigned.


Qualifications:

  • Sensitive to the needs of seniors and adults with disabilities from diverse ethnic and minority cultures.
  • Must be a senior of 60 years or above, or adult with disabilities
  • Ability to work independently with minimal supervision.
  • Must be organized and have the ability to follow through with assignments0
  • Excellent verbal communication and interpersonal skills.
  • Must be able to travel via public transportation or personal vehicle for the performance of job duties. If driving, must have and maintain a valid CA driver license, satisfactory driving record, and auto insurance coverage as specified in Self-Help's policy.
  • Must be available to work weekends, holidays and evening hours as needed.
