Who We Are:
Second Chances Comprehensive Services LLC is a caring organization with a focus on at-risk youth in the community. We provide mental health services in the Virginia communities around Hopewell, Virginia Beach, Hampton, and Emporia. Our founders recognized the need for strong community outreach services that may help facilitate and guide individuals with their future endeavors while maintaining family cohesiveness. Everything we do is about empowering our clients to have the mental health peace of mind that they need for a second chance at life.
The Qualified Mental Health Professional (QMHP) provides training and support to individuals and families within their territory. The QMHP will provide life skills coaching, case management, and other therapeutic interventions for the population served who are at risk of homelessness and or inpatient hospitalization due to their behaviors in the home and /or community. The QMHP is responsible for assisting the individual in identifying and addressing the behaviors that have made it difficult to function in the home and community. The QMHP will, with input from the individual and family, design and implement a service plan to target problematic behaviors. Continuous contact with individuals and other staff in planning and providing individual services. Regular contact with family members and members of other service agencies providing additional services to individuals such as social workers, psychiatrist, court officials and police, and other human services professionals.
