The Qualified Mental Health Professional (QMHP
) provides training and support to individuals and families within their territory. The QMHP will provide life skills coaching, case management, and other therapeutic interventions for the population served who are at risk of homelessness and or inpatient hospitalization due to their behaviors in the home and /or community. The QMHP is responsible for assisting the individual in identifying and addressing the behaviors that have made it difficult to function in the home and community. The QMHP will, with input from the individual and family, design and implement a service plan to target problematic behaviors. Continuous contact with individuals and other staff in planning and providing individual services. Regular contact with family members and members of other service agencies providing additional services to individuals such as social workers, psychiatrist, court officials and police, and other human services professionals.

Required Knowledge, Skills, Licensure, Training & Travel Requirements (if applicable):

  • Education: Bachelor’s or Master’s Degree in human services, a related field, or degree deemed equivalent to those described, from an accredited college. A human services field includes social work, gerontology, psychology, psychiatric rehabilitation, special education, sociology, counseling, and human services counseling.
  • Work experience: Considerable experience working with emotionally troubled individuals, or equivalent experience as specified by VDBHDS/DMAS regulations. Must be able to show a minimum of one (1) year direct service to individuals. QPPMH meeting VDBHDS/DMAS regulations are also considered.
  • Knowledge, Skills & Abilities: Sound knowledge of non-violent crisis intervention. Demonstrated skill in working with individuals with mental health diagnoses. Excellent oral and written communication skills. Ability to establish and maintain effective relationships with individuals receiving services and staff.
  • Interpersonal Skills: Continuous contact with individuals and other staff in planning and providing individual services. Regularly contact members of other service agencies providing additional services to individuals such as social workers, psychiatrist, court officials and police, teachers and other human services professionals.
  • Travel Requirements: A valid VA Driver’s License and car insurance required. Travel includes home visits to assigned clients and transporting clients to and from appointments and other activities as needed within the Individual Service Plan.

Essential Job Duties

  • Assists the individual and their family in implementing Individual Services Plans. 20%
  • Under the supervision of the Clinical Case Manager, provides psycho-educational, mentoring, and crisis de-escalation services, coaching/training in life skills, social skills, anger management, training in self-sufficiency and self-management skills. 20%
  • Assists in providing non-vilent crisis in intervention for individuals and families. 20%
  • Monitors and records individual behavior and documents individual progress according to existing standards and policies and to assure protection of individual confidentiality. 20%
  • Transports individuals to appointments, court hearings, work/training, and other activities as outlined in the ISP. 15%
  • Other duties as assigned/required 5%
