Sea Coast Home Health & Hospice is Hiring a Hospice, RN Case Manager Near San Diego, CA
Dear Applicants,
This Position is Monday to Friday from 9:00am-5:00pm. No rotation. No On-call required. Competitive Pay.
Sea Coast Hospice is looking for full-time RN Case Managers in San Diego area. Many of our patients are located in the same facility. We are a family owned Hospice that provides excellent individualized care. If you are a warm, caring nurse who is passionate about hospice then we would love to hear from you!
Responsibilities:
- Completes patient and family assessments to determine hospice needs
- Provides skilled nursing care for pain and symptom management
- Provides emotional support and counseling to patients and their families with support of interdisciplinary team
- Creates an individualized plan of care tailored to patient and family/caregiver needs
- Administers medications and treatments as prescribed by physician
- Provides health care instructions to the patient and family/caregiver as appropriate per assessment and plan of care
- Communicates with the physician regarding the patient’s needs and reports changes in the patient’s condition
- Teaches the patient and family/caregiver self-care techniques as appropriate. Provides medication, diet and other instructions as ordered by the physician and recognizes and utilizes opportunities for health counseling with patients and families/caregivers
- Works with interdisciplinary team to facilitate providing care that addresses physical, psychosocial, and spiritual needs of patients and family/caregiver
Qualifications:
- Registered Nurse with current unrestricted CA license
- Excellent observation, verbal and written communication skills, problem solving skills.
- Must be a licensed driver with an automobile that is insured in accordance with state/organization requirements and is in good working order
- Possesses and maintains current CPR certification.
