I. SPECIFIC RESPONSIBILITIES
Able to give immunizations and follow NYSDOH guidelines regarding immunizations.
Work within an electronic health record.
Manage and follow ACS guidelines and 29I requirements in providing care to children in foster care.
Performs physical, mental health and developmental assessments of the child at admission.
Arranges for initial and annual comprehensive physical examinations, by a medical provider.
Arranges for initial and annual dental and optical examinations by agency contracted providers
Schedules appointments with primary care providers and appropriate medical specialists.
Maintains comprehensive electronic medical record on each assigned child.
Develops a written descriptive health plan, in conjunction with the medical provider, of each child.
Oversees consent issues for procedures and treatment.
Monitors compliance ensuring youth comply with time frames for appointments and follow-up services.
Develops health histories and abstracts for a) children being transferred to another agency; or schools.
Provides discharge summaries for youth aging out of foster care.
Obtains family health history, In conjunction with caseworker, from birth parents.
Reports all illness and/or accidents, hospitalizations as required by the regulatory agency.
Contacts medical providers, hospital clinics, and laboratories for medical reports.
Attends staff meetings and case conferences when indicated.
Reviews medical records, laboratory and medical reports.
Provides psychiatrists with medical information, including laboratory results for monthly medication review.
Provides health education to youth in the practice of preventive health and safety measures.
Provides training to patients and foster parents on topics related to preventive health and safety measures.
Provides counseling to adolescents on HIV related information, family planning and other sexually transmitted infections and documents these discussions.
LPN created, implements and maintains a care plan for each identified problem.
LPN to provide daily management and coordination of appointments, supplies and medication for foster children on his/her caseload and acts as a resource to other staff members involved in patient care.
LPN receives visit summaries from all medical appointments and documents them in the record. Reviews and makes all follow up appointments. Documents in the record.
LPN provides training for foster children and staff on medical condition, health education, proper administration of medication, safety precautions including response to side effects, universal precautions, age anticipatory guidance, and any other training based on the care plan.
LPN shows competency in nursing practices: proper nursing skills, proper use of medical equipment and terminology, employs current nursing protocols and procedures.
LPN makes appropriate referrals for dental, hearing and vision screening and other problems and follows in a timely manner.
LPN maintains medical records in an updated (real-time), organized and legible condition including medications.
LPN performs other related duties as requested by supervisor/Director

Qualifications:
Experience or interest working with children in Foster care.
LPN currently licensed by the New York State Education Department.
Proof of vaccinations required.
