Your day.

As a Home Health Clinical RN Manager, you need to know how to:

  • Manage licensed and non-licensed personnel in the provision of care to patients as assigned. Conduct on-site initial clinical assessment and periodic reassessment as needed and communicate with physician, as needed. Plan, coordinate and deliver care to patient and families as appropriate. Participate in on-call coverage as required.
  • Manage daily staffing and productivity. Promote department sustainability and cost containment efforts to achieve budget. Assist director with development of annual budget, equipment needs and forecasts, and capital planning. Oversee and develop standards of care and standards of practice, and direct fiscal management.
  • Support the leadership team in developing and implementing strategies to reduce cost and improve quality of care/service.
  • Develop, implement, and monitor departmental policy and procedures which support the organizations goals and business objectives and ensure they are met. Assure complete and accurate clinical records in compliance with all regulatory agencies.
  • Evaluate associate performance and provide timely feedback and opportunities for improvement. Partner with human resources business partner to address associate relations and disciplinary issues. Utilize feedback and data from annual survey to improve associate engagement. Create an environment conducive to professional staff growth to promote recruitment, retention, and quality patient care. Coordinate and perform onboarding and orientation for new clinical associates.
  • Review, process and coordinate admissions serving as the clinical resource for the intake team.
  • Manage and execute quality improvement activities. Ensure delivery of cost effective, high quality and appropriate person-centered care. Align service goals and objectives to support department strategies. Collaborate with director to develop quality indicators and quality improvement initiatives for service line/ department. Incorporate department protocols for specific patient populations in planning practice and documentation. Design and evaluate processes to improve systems and patient care results across the continuum of care.
  • Facilitate and / or participate in care coordination and interdisciplinary team meetings in order to update patient care plans, provide an exchange of information and problem solving.
  • Support a growth culture and ensure the growth and profitability of the agency through the responsible use of company resources & education to the community regarding our services.
  • Report grievances and complaints made by patients, families, referral sources and other health care professionals. Report allegations of patient abuse, neglect and/or misappropriation of patient property.
  • Develop and monitor budgets for the appropriate use of human and material resources. Monitor financial performance and identify and implement strategies to reduce costs and improve quality of care/service.

Your experience.

We hire people, not resumes. But we also expect excellence, which is why we require:

  • Associate’s Degree in Nursing (ADN / ASN)
  • Current Colorado RN license in good standing or compact license with the ability to obtain Colorado RN license
  • Current BLS certification endorsed by the American Heart Association
  • Current State of Colorado Driver’s License
  • Minimum of three (3) years of related healthcare experience
  • Minimum of one (1) year of home health experience
  • Proven clinical acumen and experience with ability in care coordination, care planning, care management, documentation and regulatory knowledge

Preferred:

  • Bachelor’s Degree in Nursing (BSN)
  • At least two (2) years of home health experience
  • Previous supervisory experience
