Description
PERFORMANCE RESPONSIBILITIES:
  • Provides transition programming for special education students consistent with state education code requirements.
  • Coordinates and supervises work experience and job-training programs involving students in special education.
  • Counsels individuals and groups of students in special education with educational and occupational problems.
  • Assists students in special education with making realistic assessments of their abilities and in establishing educational and occupational goals.
  • Provides motivation and accommodations for students who have difficulty with the transition process.
  • Undertakes continuing professional study to improve personal knowledge and remain current on state requirements.
  • Maintains attendance records and all other records pertinent to the special education program for local, state or federal reports.
  • Participates in transition planning and preparation for everyday experiences.
  • Teach transition classes for students, ages 14-21, in ID and ASD classrooms.
  • Assists students, as needed, in the acquisition of basic employment skills.
  • Provides job placement services to students, ages 14-21, based on educational need at job sites in the community and within the school district.
  • Serves as a liaison between community transition agencies and special education staff.
  • Trains and directs job coaches to support transition programming.
  • Consults with and assists the Director of Student Services and Building Principal as needed.
  • Fosters professional competence through in-service, workshops, and other professional development activities.
  • Participates in the IEP process as appropriate and as required by law.
  • Participates in faculty meetings, committees and the sponsorship of pupil activities.
  • Maintains professional competence through in-service education activities provided by the district, and in self-selected professional growth activities
  • Educate parents by distributing information on transition issues, hosting parent information meetings, and scheduling tours of adult facilities.
  • Performs all other duties as assigned by Administration.

  • Effectively plans using approved curriculum, instructional strategies, resources, and data to meet the needs of all students.

3. Instructional Delivery

  • Effectively engages students in learning by using a variety of instructional strategies in order to meet individual learning needs.

4. Assessment For and Of Learning

  • Systematically gathers, analyzes, and uses relevant data to measure student progress, guide instructional content and delivery methods, and provide timely feedback to students, parents, and stakeholders.

5. Learning Environment

  • Uses resources, routines, and procedures to provide a respectful, safe, positive, student-centered environment that is conducive to student engagement and learning.

6. Professionalism

  • Demonstrates behavior consistent with legal, ethical, and professional standards, contributes to the profession, and engages in professional growth that results in improved student learning.

MINIMUM QUALIFICATIONS:

  • Valid license in a Special Education from the Wisconsin Department of Public Instruction (DPI).
  • Bachelor's degree.

PREFERRED QUALIFICATIONS:

  • Experience in coordinating transition supports across education settings at the secondary level.

PHYSICAL DEMANDS:

1. Capable on a continuous basis of standing, sitting, reaching, grasping, bending, stooping, twisting; repetitive movement of both hands and feet; seeing with both near and far visual acuity and to hear.

2. Capable on a frequent basis of walking and moving, climbing stairs, lifting from floor to waist (up to #50), waist to shoulders (up to #25).

3. Capable on an occasional basis of pushing; pulling; lifting from shoulders to overhead (up to #10).
