SUMMARY OF DUTIES:
As a direct support professional, the Direct Support Professional is responsible for delivering quality services while maintaining a safe and therapeutic environment that supports the emotional and physical well-being of people receiving supports. The Direct Support Professional is committed to a person-centered approach and supports the activities outlined in the individual’s plans of service. The Direct Support Professional will encourage people to lead self-directed lives, including active participation in their community to the greatest extent possible.
ESSENTIAL DUTIES:
1. Responsible for providing supports, supervision, assistance or skill acquisition as outlined in the person’s service plans. These areas may include, but not limited to personal hygiene, housekeeping, shopping, and activities/events within the community both vocational and skill-based activities.
2. Responsible for service recipients’ safety in the home, worksite, and community, as outlined in protective oversight plans or identified safeguards in site-based programs. Assists in monitoring and providing medical/health supports as directed by medical staff.
3. For site-based programs, completes routine housekeeping and maintenance which may include, but not limited to, cooking, cleaning, shoveling and gardening; production/activity implementation.
4. Attends all scheduled meetings and trainings.
5. Completes required reporting, recording and service documentation for each day.
6. Participates in training modules. Applies practices per training and demonstrates knowledge of service standards, rules and regulations.
7. Responsible for transporting, accompanying, and assisting individuals in the community, and/or on appointments as needed.
8. Recognizes and immediately reports any safety concerns, including fire safety, physical plant, vehicles, safety of individuals, staff or others.
9. Notifies supervisor when agency, household and/or program supplies are needed.
10. Models expected behavior in the community, at the worksite, and at recreational activities. Maintains professionalism at all times. Adheres to the DSP Code of Conduct.
11. Responsible for ensuring that all confidential and potentially sensitive information is processed, retained and utilized in a strict and confidential manner.
OTHER DUTIES:
12. Adheres to all agency policies and procedures.
13. Responsible for displaying sound, ethical judgment and supportive decision making as outlined in the DSP Code of Ethics.
14. Participates in and provides feedback and recommendations in quality improvement activities for services provided, as well as overall agency systems and processes.
15. Other duties as assigned.
QUALIFICATIONS:
Must be at least 18 years old. A valid New York State Driver's License with driving record that is acceptable within sasi’s criteria will be required. Any exceptions to this requirement will be based on assignment and at the sole discretion of sasi President & CEO. Must be able to perform each essential duty satisfactorily. NOTE: In addition to meeting OPWDD’s background check, may also be required to meet other background checks, such as, OMH and Office of Children and Family Services background checks.
