POSITION SUMMARY:

The Clinical Pharmacist is responsible and accountable for the provision of safe, effective, and prompt medication therapy. Through various assignments within the department, they provide support of centralized and decentralized medication-use systems as well as deliver optimal medication therapy to patients with a broad range of disease states.

DUTIES AND RESPONSIBILITIES:

1. Leads assigned patient service lines, clinical areas, and therapeutic programs
2. Facilitates pharmaceutical care services, direct patient care programs, medication utilization systems within assigned services and care areas to assure drug utilization activities are aligned with patient care needs, evidence-based best practices, and regulatory standards
3. Designs and implements stewardship activities and restriction/surveillance programs
4. Tracks and evaluates assigned pharmacy programs for operational, quality, and financial efficiency and routinely benchmarks against local and national best practices
5. Actively identifies practice related issues which require evaluation and facilitate clinical research projects, quality improvement initiatives, or healthcare provider education as needed to advance practice
6. Develops and oversees policies and procedures for drug purchasing, drug usage, drug distribution, and drug control
7. Assures pharmacy is an integral part of the health-care delivery system and facilitates enhancement and expansion of pharmacy services/programs
8. Delivers direct patient care and clinical practice, including decentralized and service-based programs
9. Maintains proficiency in decentralized pharmacy services and clinical pharmacy programs
10. Works as an active member of multidisciplinary team and collaborates with healthcare providers within decentralized patient care areas to provide patient-centered care
11. Identifies high-risk patients and implements interventions to improve quality and safety
12. Makes appropriate evidence-based, patient-centered medication recommendations
13. Participates in the management of medical emergencies
14. Provides discharge medication review, reconciliation, and counseling as appropriate
15. Provides pharmaceutical services throughout medical center
16. Maintains proficiency in hospital computer systems and medication ordering systems
17. Provides accurate, safe, timely, and appropriate medication therapy based on patient age and needs
18. Completes critical patient monitoring and reviews patient profile/chart to identify, prevent, or mitigate drug-related problems, improper drug or dose selection, sub therapeutic dosage, over dosage, adverse drug reactions, drug interactions, failure to receive drugs, untreated indications, medication use without an indication, and treatment failures
19. Identifies, designs, and implements improvements in the medication-use system to advance patient safety, maximize therapeutic outcomes, or control costs
20. Delivery of pharmacy care in accordance with the Catholic Moral and Ethical Directives. 
21. Follows all HIPAA and OSHA guidelines and regulations, including assisting HIPAA and OSHA Officers in ensuring compliance.

Part-Time: 24 hours a week

Requirements

QUALIFICATIONS, EDUCATION AND EXPERIENCE:

1. Must have a B.S. in Pharmacy or Pharm D from an accredited School of Pharmacy or foreign equivalency and a current Texas License.
2. Must have a minimum of two-year experience as a clinical pharmacist or completion of ASHP accredited PGY-1 Pharmacy residency.
3. Must have an active Texas preceptor license and be certified in administering immunizations.
4. Can demonstrate excellent organizational skills, multi-tasking and effective use of time. Also able to handle and complete multiple tasks or projects with multiple deadlines. 
5. Ability to deal professionally, courteously and efficiently with public and all levels of the organization.
6. Ability to operate computer, copier fax, and scanners.
7. Must be proficient in Microsoft Office, practice management system software applications, and EMR.
8. Can demonstrate effective verbal and written communication skills.
9. Can demonstrate effective customer relation skills, working with diverse populations, often in stressful fast paced environments.
