San Francisco Community Health Center is Hiring a Street Medicine Program Manager Near San Francisco, CA

The Street Medicine program serves people who are either experiencing unsheltered homelessness or unstable housing. The multidisciplinary team (provider, nurse, case manager, medical assistant, therapist, community health worker) provides high quality medical care and social services using a street medicine approach.The Street Medicine Program Manager, in collaboration with the Street Medicine Associate Director of Street-Based Services, will provide day-to-day oversight, supervision, and coordination over the Street Medicine program. The Program Manager will also be responsible for directly supervising team case managers, community health workers, and the Eligibility and Referral Coordinator. They will also provide direct patient care and case management as part of the street medicine care team. This role will report to the Associate Director of Street-Based Services.Essential Duties:Administrative Functions:Serves as day-to-day contact person for all program staff.Reviews program objectives and performance and designs strategies to improve service delivery, especially as related to non-clinical patient care.Ensures that all direct service staff provide strengths-based, trauma-informed, and culturally competent services to clients.Delivers or arranges trainings and resources required for direct reports and direct service staff to perform their roles, especially as related to non-clinical patient care.Ensures program deliverables are being met.Develops and maintains protocols to ensure client engagement in care.Develops, maintains, and supports schedules of SFCHC street medicine staff.Attends and facilitates weekly case conferences, and daily team huddles.Hires, supervises, evaluates, and professionally develops direct reports.Oversees the delivery of high-quality patient-centered care plans by the Street Medicine program staff.Assists with other administrative duties as needed.Patient Care Functions: Works collaboratively with the Street Medicine team and clinical collaborators at local hospitals and community partners to connect unhoused people with the SFCHC street medicine team when appropriate.Locates and builds relationships with unhoused people in need of primary care.Documents appropriately in the Electronic Medical Record.Uses multiple electronic data sources to gather patient information and track ED or inpatient utilization across care systems.Performs case management assessments, implements plans of care, and arrange for appropriate follow-up care for primary care patients in both non-traditional and traditional settings.Provides team-based medical case management according to SFCHC and Enhanced Care Management protocols.Performs initial and continued assessment of clients Social Determinants of Health and develops care plans prioritizing a patients specific barriers to health and wellness.Makes appropriate referrals to local social services agencies to address a patients Social Determinants of Health.Provides culturally and racially competent client care in a manner that is both non-discriminatory and non-judgmental.Communicates effectively with client/patient, clinic staff, the public at large and departmental staff in a non-discriminatory manner.Conducts and/or supervises administrative aspects of care coordination.Helps to coordinate medication pickups/drop-offs and specialty appointment accompaniment for street medicine patients, in collaboration with program clinical staff.Serves as an integral member of the interdisciplinary team to provide wrap-around services/care for clients in a team-based care model.Travels on foot around the Tenderloin neighborhood and surrounding area.Carries outreach supplies in a backpack to patients within the community.Minimum Qualifications: Bachelor's degree preferredAt least 3 years of experience that can be demonstrated to be applicable to the duties listed in the job description, including some experience with providing care in nontraditional settings.Effective written and verbal communication skills.Must have a philosophy that is consistent with the Mission, Vision, and Core Values of SFCHC.Ability to organize, plan, and effectively work on an interdisciplinary team.Demonstrated experience supervising and leading teamsKnowledge of harm reduction and trauma informed approaches to client care.Familiarity with issues faced by the transgender community, sex workers, and substance usersKnowledge of HIV transmission and prevention. Knowledge of health and social issues related to HIV transmission in high-risk populations.Ability to work with and maintain boundaries with clients and confidential informationStrong written and verbal communication skillsStrong organization skills and ability to apply new knowledge into day-to-day client workProficiency in Microsoft Office ProgramsSelf-directed, motivated and flexible with the ability to work independently and as part of a teamFully vaccinated against COVID-19, complete with booster, and able to show proof of vaccinationUp-to-date with vaccinations, especially seasonal flu shot vaccineProof of TB clearance (issued within the past 12 months or less)Preferred Qualifications: Bilingual and bicultural1 year of direct supervision experience1 year of experience with program designProfessional experience working with people experiencing unsheltered homelessnessLocation: San Francisco, CA#J-18808-Ljbffr
