San Francisco Community Health Center is Hiring an Associate Director of Integrated Care Near San Francisco, CA

Taimon Booton Navigation Center (TBNC) is a Trans specific Navigation Center / shelter.

The Associate Director of Integrated Care will oversee the coordination and integration of whole person care at TBNC. The Associate Director is responsible for the direct supervision of Case Managers. The Associate Director will collaborate with the Associate Director of Guest Services to achieve program goals and objectives. Associate Directors will also be responsible for the management of Supervisors on shift. Associate Directors will also be expected to employ harm reduction and de-escalation techniques in day-to-day work. The Associate Director of Integrated Care is an on-site, in-person position and reports directly to the Site Director.

Essential Duties:

· Oversee the day-to-day functions of the integrated care team including supervision of case management team at TBNC
· Assess the whole person needs of guests and staff
· Oversee whole-person, onsite care including the coordination and integration of medical and behavioral health services
· Develop and cultivate community partnerships related to in-scope services, especially clinical and behavioral health community partnerships
· Hire, supervise, retain and evaluate Case Managers, Navigators, and other ancillary staff, as needed
· Develop and maintain schedule of Case Managers, including requests for visits needed offsite
· Ensure that integrated care team has all resources needed to fulfill the job duties, including trainings, and strong relationships with needed outside resources|
· Attend case conferences and lead staff with task management and follow up
· Create systems for tracking data, including Onesystem compliance
· Assist in the preparation of case management documents for yearly audit
· Ensure progress is being accomplished toward specified program objectives
· Create, organize, and implement project timelines, calendars and schedules
· Create healthy boundaries with guests and staff, pass down rules and guidelines as they are in the SFCHC handbook; hold staff accountable to the set expectation
· Support and guide Supervisors on shift
· Respond to crisis on site
· De-escalate guests and/or staff situations as they arise
· Report behavioral issues, allegations or other issues for concerns to Site Director immediately
· Work on weekends and/or holidays as needed.
· Assist with other administrative duties as needed

Clinical Essential Duties:

· Conduct intake, assessment and reassessment of clients with mental health and substance use disorders
· Offer individual and/or group clinical supervision regarding trauma response, overdose, de-escalation and/or other situations appropriate to the job
· Continually update client care plan goals and develop appropriate discharge plans whenever needed
· Provide service referrals, linkages, care coordination, and advocacy services for clients, and monitor the outcomes of these activities
· Provide onsite consultation to the medical clinic and other programs for urgent behavioral health crisis situations
· Complete necessary documentation and progress notes
· Participate in case conferences, monthly staff meetings, consult meetings and other necessary in-service trainings
· Participate in and comply with agency quality assurance, peer review, clinical consultation
· Work collaboratively with an interdisciplinary team of medical services, medical case management, prevention and testing services, and other internal and external social support programs

Minimum Qualifications:

· Graduation from an accredited Masters of Social Work program.
· ASW registration required or equivalent required; license is preferred
· At least one year of experience providing program management or coordination in a community-based case management program
· Experience working with people of different racial and ethnic backgrounds, genders identities, sexual orientations, people who use substances, including injection drugs, those experiencing homelessness, people with mental health challenges, people with disabilities, and people living with HIV/AIDS and/or Hepatitis C
· Sensitivity and familiarity with HIV/AIDS and cultural issues in communities of color.
· Experience working in interdisciplinary teams.
· Demonstrated reliability and good interpersonal, organizational, verbal, written communication skills in English. Must be able to handle multiple tasks and work professionally and ethically within multi-racial and multicultural settings.
· Be able to provide customer service effectively, courteously and friendly and meet the needs of diverse clientele.|
· Ability to travel within San Francisco
· Some evening and weekend hours are required.
· Fully vaccinated against COVID-19, complete with booster shot, and able to show proof of vaccination.
· Up-to-date with vaccinations, especially seasonal flu shot vaccine
· Proof of TB clearance (issued within the past 12 months or less)

Preferred Qualifications:
· Spanish language capacity
