As an On-Call Registered Nurse (RN) in Home Health, you will be an essential part of our healthcare team, providing skilled nursing care to patients in their homes. You will be responsible for delivering high-quality, compassionate care to patients who require assistance with various medical needs, ensuring their comfort and well-being.

Key Responsibilities:

• Respond promptly to on-call requests for nursing assistance during after hours and weekend coverage times.
• Conduct comprehensive assessments of patients' health status and develop individualized care plans for SOC and ROC over the weekend.
• Administer prescribed medications and treatments as per physician orders during visits per POC.
• Monitor patients' vital signs and observe for any changes in their condition.
• Provide wound care, medication management, and other skilled nursing procedures as needed.
• Educate patients and their families about disease management, treatment plans, and self-care techniques.
• Collaborate with interdisciplinary healthcare team members to ensure coordinated and effective patient care.
• Document all nursing interventions, assessments, and patient responses accurately and timely.
• Maintain confidentiality of patient information and adhere to HIPAA regulations.
• Participate in ongoing training and professional development activities to stay updated with best practices and industry standards.

Qualifications:

• Valid Registered Nurse (RN) license in Texas.
• Previous experience in home health or a similar setting preferred.
• Strong clinical assessment and critical thinking skills.
• Excellent communication and interpersonal abilities.
• Ability to work independently and as part of a team.
• Reliable transportation and willingness to travel to patients' homes.
• Flexibility to work rotating shifts, including evenings, weekends, and holidays.
