- Assess patients' physical, emotional, and spiritual needs in the hospice setting.
- Develop and implement individualized care plans to address patients' unique circumstances.
- Collaborate with interdisciplinary teams to ensure comprehensive and holistic care.
- Provide pain and symptom management, adjusting care plans as needed.
- Educate patients and families on the hospice process, treatment options, and supportive care.
- Monitor patient progress and adjust care plans accordingly.
- Document patient care accurately and maintain up-to-date medical records.
- Registered Nurse (RN) with an active and unrestricted license.
- Previous experience in hospice, palliative care, or a related field is advantageous.
- Strong clinical skills and knowledge of end-of-life care principles.
- Excellent communication and interpersonal abilities.
