The Housing Case Manager will provide case management to participants experiencing homelessness.
The CM follows established evidenced based best practices to create a safe and stable environments for participants; the CM’s knowledge and understanding of relevant resources is intended to be housing-focused, trauma informed and oriented toward partnering with families and/or individuals by coming along side and walking them toward their goals. This position requires a high degree of professionalism, commitment and adaptability in order to effectively uphold community values while maintaining an environment of dignity, respect and collaboration with participants and partners.
As a housing navigator, you will leverage your passion for public service to help the agency’s clients find affordable and safe housing in great neighborhoods throughout the county. You will work closely with authority personnel, landlords, and organizations that have an interest in housing expansion.
Skills and Abilities: Ability to work with minimal supervision and exercise good judgment; strong verbal and written communication skills; data management, problem-solving skills, and critical thinking skills, interpersonal skills; ability to follow assigned workflows and procedures; ability to handle multiple tasks and manage competing demands; standard office computing skills required.
Essential Duties/Responsibilities: To perform this job successfully, an individual must be able to perform each essential duty satisfactorily. Reasonable accommodations may be made to enable individuals with disabilities to perform the essential functions.
1. Maintain a case load of up to 20 individuals/families and work towards goals and create case plans.
2. Work collaboratively with participants to help address issues that may impede access to housing
3. Practice core principles of Housing First, Trauma Informed Care and Motivational Interviewing
4. Complete required assessments with clients as part of program intake procedures.
5. Provide strengths-based, flexible and responsive case management supports to participants
6. Partner with participants to identify their goals/visions for themselves and children
7. Develop housing stability plans based on identified goals/visions for both adults and children related to obtaining or maintaining housing, income or other service needs
9. Facilitate housing placement process for each participant and serve as liaison between families, landlords, utility providers and other agencies when needed
10. Assist with move-ins to housing to ensure the participant has necessary household items and basic understanding of their responsibilities as a tenant
11. Provide participants with connections to services and resources that may support their safety, well-being and/or achieve their long-term stability
12. Network with community resources for housing, employment, substance abuse treatment, mental health treatment, training, and make referrals as appropriate
13. Obtain signed release of information forms and communicate regularly with staff at treatment programs, and vocational programs;
14. Complete data entry in the Homeless Management Information System (HMIS); data entry includes case notes within five (5) business days of contact, self-sufficiency assessments and related participant information and updates
15. Maintain accurate and detailed case files (both physically and electronically) on each participant including copies of all intake documents, case plans, monthly budgets, and other program forms
16. Participate in registry and service coordination meetings, case conferences and training programs
17. Demonstrate effective communication skills in building relationships with peers and participants
18. Other duties as assigned in support of CES operations and the implementation of rapid rehousing
Education and Experience Candidates must possess an Associate’s Degree or equivalent number of college credits in a social services field. A Bachelor’s Degree is preferred. Degrees from other disciplines will be accepted given the candidate provides evidence of prior experience that would allow them to competently perform the primary duties listed below. Candidate should have at least 2 years of documented case management experience.

Physical Demands While performing the duties of the job, the employee is regularly required to communicate in person or by telephone.The employee must sit for extended periods of time.
Work Environment: The position involves attention to detail and extensive organizational skills. The employee will interface with clients, agencies and funders. The employee will likely encounter and be expected to address challenges associated with clients’ experiences with long-term homelessness, mental health and substance abuse concerns, and other barriers to obtaining decent, affordable housing.
