Responsibilities: - Provide case management services to individuals in need - Conduct assessments to determine client needs and develop appropriate care plans - Collaborate with clients to set goals and monitor progress - Coordinate and connect clients with community resources and support services - Advocate for clients' rights and access to necessary services - Provide addiction counseling and support for individuals struggling with substance abuse - Assist clients in navigating the Section 8 housing program - Utilize social work principles to address clients' psychosocial needs - Assist clients in understanding and accessing Medicare benefits - Mentor and support clients in developing life skills and achieving personal growth - Supervise and monitor client progress, ensuring compliance with program requirements - Provide crisis management and intervention services as needed - Collaborate with other professionals, such as behavioral health specialists, to provide comprehensive care - Support workforce development initiatives by assisting clients in finding employment opportunities Skills: - Strong knowledge of addiction counseling principles and techniques - Familiarity with the Section 8 housing program and its requirements - Proficient in social work practices and ethics - Understanding of Medicare benefits and eligibility criteria - Ability to provide mentoring and guidance to individuals in need - Experience in supervising and monitoring client progress - Knowledge of behavioral health issues and interventions - Proficient in crisis management techniques and crisis intervention strategies - Familiarity with workforce development programs and resources This position requires excellent communication skills, empathy, and the ability to work collaboratively with diverse populations. A bachelor's degree in social work or a related field is preferred, along with relevant experience in case management or a related field.
