SAFREST is Hiring a RN Clinical Manager Near Middleburg, OH

The Clinical Manager is a Registered Nurse (RN) who is responsible for:
 Case management for skilled and non-skilled home care cases
 Coordination and delivery of patient care
 Quality of services provided
 Training and supervision of skilled and non-skilled staff
 Meeting and exceeding Joint Commission accreditation standards
The person in this role assures that care is coordinated between important stakeholders (i.e., clients and family members, healthcare providers, and our agency) and that all the client’s needs are identified through the assessment process and fulfilled through the implementation of the Plan of Care and supervisory process. Also, this person establishes, implements, and evaluates clinical goals and objectives for home care services that meet or exceed our standards of quality.

Oversight and execution of client plan of care. Meet exceed Joint Commission accreditation standards.

company, clients, one another.
Responsibilities include, but are not limited to…
Case Management/Patient Care:
 Conduct initial admission assessments, re-assessments, and safety evaluations for home care clients
 Provide patient care as defined within the scope of the Nurse Practice Act - Ohio Revised Code (ORC), such
as Infusion Therapies (e.g., IV/IG), wound care, injections, and more
 Create and maintain plans of care within a person-centered framework
 Obtain physician signatures for all skilled plans of care
 Coordinate care with other providers, clients and their families, and care staff to ensure full understanding
of client’s conditions, changes in condition, and overall needs
 Manage referral intake for skilled cases and follow-up with referral sources, clients, families, other care
providers, etc.
Primary Administrative:
 Conduct chart reviews weekly to ensure ongoing compliance with state regulations, Joint Commission
(JCAHO) and brand standards, including but not limited to signed physician's orders, authorizations, and
supervision over high-risk medications
 Manage incident reporting (i.e., reviewing and follow-up as needed to ensure positive resolution and
future risk mitigation)
 Daily review of nurse's notes and other documentation to ensure all required notes are complete and
turned in timely to avoid any delays in invoice submission or claims reimbursement
 Complete timely discharge of clients with a thorough discharge summary
 Assure proper maintenance of client clinical records, statistics, reports, and records for purposes of
evaluation and reporting of organization activities
 Lead Joint Commission accreditation and survey readiness
 Understand and uphold existing agency policies and procedures
 Recommend, develop, and administer new agency policies and procedures
 Supervise, educate, and coach other nursing personnel/caregiving staff
o Assess competency of clinical staff and ensure staff is assigned in a manner that promotes quality,
continuity, and safety of a client’s care
o Complete staff supervisory visits to ensure compliance
o Conduct field staff education and ongoing training (both skilled and non-skilled), including but not
limited to weekly orientation, skills competency, and basic nursing skills and documentation
o Plan and implement in-service and training programs
 Participate in after-hours on-call rotation (phone only, typically one week on, four weeks off)
 Ensure availability to manage critical clinical issues
 Conduct caregiver and client introductions as needed
 Attend and participate in office meetings as needed
 Support talent acquisition needs of agency to build a strong bench of competent care staff
 Stay informed about changes in the field of nursing and home care and share information with other
Independently Owned and Operated agency stakeholders
Qualifications & Requirements:
 Current unrestricted/unencumbered Registered Nurse (RN) license in the State of Ohio
 Degree in Nursing from a National League of Nursing accredited program
 Minimum three (3) years of clinical experience with at least one (1) year of documented case management experience
 Minimum one (1) year direct supervisory experience
 Preferred: At least two (2) years experience in home health
 Preferred: At least one (1) year experience working with pediatric cases
 Ability to travel within the assigned territory daily including weekends, nights, and holidays
 Must have flexibility in schedule to be on call 24/7 as needed
 Understand regulatory compliance with state and federal regulations
 Familiarity with preparing for Joint Commission or similar accreditation surveys
 Adhere to HIPAA and maintain client confidentiality
 Possess exceptional nursing assessment skills
 Proven ability to build quality and implement improvements within the operation
 Attention to detail possessing strong organizational skills
 Excellent customer service skills and ability to deal tactfully with clients, staff, and community
 Ability to multi-task, troubleshoot and problem-solve in stressful situations
 Strong interpersonal skills, including active listening, good communication, leadership, motivation, and empathy
 Ability to manage complaints and difficult situations requiring critical thinking
 Excellent verbal and written communication skills (must be comfortable with public speaking)
 Ability to give and take direction, absorb information quickly, and work independently or within a team
 Comfortable with technology (i.e., strong PC skills including Microsoft Office applications and Google)
 Valid driver’s license, reliable transportation, and insurance
 Current CPR Certification
 Negative TB test results in past 12 months
 Up-to-date vaccinations including Influenza, COVID-19, Tetanus, Diphtheria, Pertussis, Rubella, Varicella, MMR, and Hepatitis B
 Ability to pass drug, background, and criminal record checks
 Ability to travel daily including weekends, nights, and holidays
Environment:
Contact with clients under a wide variety of circumstances. Care provided in client’s living environment with varying situations or in an institution such as a hospital. May be required to respond to emergency situations. Travel required. Position includes tasks that have the potential for exposure to blood and other potentially infectious material (OPIM) such as body fluids/tissues.
 Office Environment: High-paced office/branch environment; continuous phone use/ringing; talking; various noise levels; interactions with employees and clients; sitting and standing for long periods of time; bending and stooping required; lifting up to 10 lbs. regularly and up to 50 lbs. may be required; using a computer for extended periods: typing, staring at a bright monitor or TV screen; using fingers, wrists, and arms; stress on back may occur; possible exposure to prescription drugs, bodily fluids, small appliances for cooking, cooling, and heating foods or securely storing medications.
 Client’s Environment: Unknown environments; bodily fluids, household and human germs; extreme cold or hot
temperatures in or outside; light to heavy traffic; inclement weather; human body weights that vary based on client health and lifting as prescribed or needed per plan of care; 50 lbs.
Personal Protective Equipment:
May include gloves, mask, eye/face protection, and disposable outer covering or lab coat.
Physical Requirements:
Must be able to stand walk, squat, bend, kneel, reach, twist, push and pull. Varied assistance is required depending on the ability of the client. Must be able to lift up to 50 lbs. Visual and hearing acuity required.
Travel Requirements:
 Travel within assigned territory and surrounding region as needed for various staff and client care service needs
 Overnight travel as directed based on the needs of the business (i.e., attending job required training and work conferences.)
