Job Summary:

• Provides direct and indirect primary nursing care to assigned participants in assisted-living environments.
• Conducts a thorough and precise assessment of patient condition, functional abilities, and environment and social factors.
• Makes appropriate referrals for additional services such as PT, OT, MSW, and HHA.
• Provides care in accordance with physician orders, the patient's condition, accepted standards of practice and Agency policies.
• Identifies changes in patient conditions; consults with patient care team and implements appropriate interventions.
• Develops and implements nursing care plans and discharge plans with involvement of patient and patient care team.
• Completes paperwork in an accurate and timely manner, including OASIS.
• Understands and demonstrates commitment to the company's mission through daily communication with participants, families, community, and staff. 
• Builds a trusting relationship with each participant/caregiver, by being available, listening, communicating, and educating via every available forum.

Requirements:

• Must be a Registered Nurse in the state of New York
• Must have home care experience 
• Computer experience required
• Bachelor's degree preferred
• Bilingual is a plus!
