OVERVIEW: The Intervention Specialist functions as the frontline worker for families who are experiencing a situation of instability, who feel overwhelmed and/or does not have the knowledge of resources when faced by challenges that divert the family’s focus away from effective parenting. This position is responsible for intervention and short-term community-based advocacy on behalf of families, including resource and referral services and actively participates in the multi-resource team.

DUTIES:

• Provides intervention and short-term case management services, which connect families to services.
• Performs assessment via phone or in-person to determine level of need, engages in intervention, and refers families to inter-agency programs or outside community resources based on the situation.
• Works closely with various community partners and program components and establishes both formal and informal points of contact with area service providers to ensure a network of support for families.
• Ensures families are connected to services.
• Provides follow-up visits to families and additional services, as needed.
• Facilitates stress-reducing activities and classes.
• Meets individually with parents experiencing high levels of stress.
• Arranges for parents in crisis to utilize SCH Crisis Nursery for respite/day care when determined appropriate.
• Refers and transitions families to effective parenting education workshops and services and provides support for assisting with attendance.
• Develops and maintains working relationships with community partners and service providers, including Child Protective Services
• Maintains working knowledge of current resources and resource mapping to provide appropriate and timely referrals for service.
• Attends and participates in all training, team meetings and team building activities.
• Participates as a member of the FRC Multi-Resource Team (MRT), and presents cases as needed.
• Provides feedback to Program Manager, Team Leaders, FRC Coordinators, Home Visitors and community partners as follow-up to families’ intervention resolution.
• Participates in Team Decision Making (TDM) process with CPS, as needed.
• Facilitates CAP training as needed.
• Completes all required documentation, case notes, MAA time surveys and data collection in a timely manner.
• Attends community meetings as necessary to make and maintain community contacts.

LOCATION: 2251 Florin Rd., Sacramento, CA 95822 (Family Resource Center - Meadowview)

SCHEDULE: Monday – Friday, 8:30 am – 5:00 pm

SALARY: $20.22 - $22.74 per hour, Non-Exempt

QUALIFICATIONS

Education and Experience:

High school diploma or GED required.

A Bachelor’s degree in social work, human services, or a related field is preferred.

2 years of experience working with at-risk families as a support worker is required.

Experience working with culturally diverse communities and families is preferred.

Driving is an essential function of this job. As such, employees in this position will be required to furnish their own vehicle to be used in the course of employment. A valid driver’s license and minimum insurance coverage is also required. Must be at least 21 years old.
