The Wound Care Coordinator (RN/LPN) will:

• Direct the overall management of all wound care in the facility
• Be responsible for the provision of treatments to patients with updates to the physician and other members of the clinical team
• Complete required record keeping upon resident admission, discharge, transfer; chart and document per facility policy
• Educate peer staff in proper wound care and serve as a resource for the clinical team

Wound Care Coordinator (RN/LPN) must have the following:

Licensed Practical Nurse (LPN)/ Registered Nurse (RN)

• Wound Care Certification: 1 year
• Long-term care experience
• CPR Certified
• Promote and facilitate teamwork
• Demonstrate a high standard of ethics
• Leadership and supervisory skills
• Able to make independent decisions
• Demonstrate excellent customer service
