Monmouth Medical Center-Southern Campus located in Lakewood is looking for an experienced Case Manager to join our great team!!

RESPONSIBILITIES:

The RN Case Manager will be responsible for all aspects of Case Management for Inpatients to determine appropriateness of continued stay, care planning, utilization management and discharge planning,

Requirements:

Current license as a registered Nurse in the State of NJ

3-5 Years of Nursing experience required

Utilization review and discharge planning experience preferred

Case Management certification preferred

On call holiday 1-2 per year and weekend rotations approximately every 6 weeks
