Summary:
Actively participates in a team-orientated inpatient and outpatient diabetes program. Responsible for individual patient assessment, education, follow-up and group diabetes teaching. Performs total direct and indirect care based on the individual's professional discipline's guidelines and the American Diabetes Association standards of practices in the clinical setting. Demonstrates the ability to deliver holistic care, while recognizing the patient's right to individualized care and treatment. Assists with the development and implementation of the Center's objectives and works cooperatively to attain them. Exemplifies the Rush mission, vision and values and acts in accordance with Rush policies and procedures.
- Conducts a comprehensive assessment of each patient to identify health status, care and educational needs based on the age, mental, physical, emotional, and financial abilities. Educates based on this assessment.
- Develops and individualizes Plan of Care with each patient based on assessment, medical condition and input from team members (physicians, nurse, and nutritionist). Utilize standards of Nursing Care and/or Nutrition Practice as well as the
- American Diabetes Association according to established guidelines.
- Evaluates the effectiveness of the Plan of Care on an ongoing basis and make appropriate chan·ges.
- Educates patients, family and significant others in diabetes management based on their professional discipline.
- Educates and supports patients with follow-up phone calls.
- Assists patients with medication anp/or dietary changes based on their professional discipline guidelines.
- Monitors each patient's medical conditions and general health status and reports all pertinent changes to physician and other appropriate personnel.
- Identifies learning needs and readiness to learn of each patient and or family and plans ways to meet and teach these needs with appropriate teaching techniques.
- Identifies potential life-threatenin situations and initiates appropriate intervention.
- Initiates referrals and collaborates with appropriate resource personnel to facilitate the delivery of care.
- Utilizes available resource materials and assists in developing others to assist in the teaching/learning process.
- Develops, updates and maintains knowledge and skills in diabetes management: meal plan, medications, exercise regimen, pattern control, monitoring devices and medication delivery systems.
- Participates in hospital and community education by presenting seminars, writing articles and hosting health fairs.
- Actively participates in the orientation of personnel and students. Acts as a resource to all staff and students.
- Participates in interdisciplinary committees as requested.
- Assists with the development and attainment of all Center objectives.
Required Job Qualifications:
- Bachelor's Degree
- RN, CDE or CDE in training with 3 years' experience
- RD, CDE or CDE in training with 3 years' experience
- Current IL license in their respective field(s)
- Certified Diabetes Educator or in training for certification (certification must be obtained within 2 years from hire)
- Excellent analytical, interpersonal skills. Knowledgeable with Epic. Excellent verbal and nonverbal communication skills
- Ability to develop presentations to meet patient/family and/or group needs Public Speaking Teaching skills
- Knowledgeable with all insulin delivery systems: injectables, pumps and inhalers; Sensor-insertion and downloading and BMR devices.
