Seeking a FULL TIME Palliative Care Team Manager/Coordinator in our RESEDA office
MUST HAVE AN MSW/LCSW PREFERRED OR AN LVN OR RN LICENSE IN CA- EITHER A SOCIAL WORKER OR A NURSE
Leadership experience is preferred in a health care setting, end of life/palliative experience preferred 
Must be computer savvy

POSITION SUMMARY
The Palliative Care Coordinator (PCC) is an experienced professional who initiates and coordinates the palliative care plan and coordinates the services provided to palliative care patients. The PCC participates in patient care conferences and works cooperatively with the attending physician, Palliative Care Medical Director, and other members of the palliative care team. The PCC provides coordination of activities of assigned RNs, LVNs and NPs. The PCC provides coordination of services to patients who have been admitted for Palliative Care services. The PCC regularly coordinates referrals to the palliative care program, assures proper authorization for care, assures visits are scheduled as authorized and that documentation by care team is conveyed to PCPs and authorizing managed care groups.
REPORTS TO: Vice President of Operations
SUPERVISES: Skilled Nurses, NPs and Unlicensed Supportive personnel
INTERACTS WITH: Assigned Social Worker, Spiritual Counselor, other services such as Medical Director, Nurse Practitioner, Skilled Nurses, PCP, Case Managers and Quality Assessment/Performance Improvement staff.
QUALIFICATIONS:
Education: Graduate of an accredited school.
Credentials: Current
Experience: Minimum of one-year experience in hospice, homecare or Palliative Care setting.
Core Competencies: Demonstrated competency in performance of professional functions of coordination of a team. Demonstrated knowledge of palliative care concepts. Ability to apply professional knowledge to the coordination of Nursing, Medical Professionals, and other members of the palliative care team. Requires excellent oral and written communication skills. Possesses a professional and sympathetic attitude toward caring for the palliative care patients and families and demonstrates positive communication skills in interacting with other members of the palliative care team.
Other: Valid driver's license, and auto insurance, reliable/well maintained transportation.
GENERAL ESSENTIAL FUNCTIONS & RESPONSIBILITIES:
1. Assures that all palliative care referrals are processed timely
2. Assures that authorization is obtained for all palliative care admissions as well as ongoing re-authorization
3. Assures that palliative care visits are scheduled as authorized by all palliative care disciplines.
4. Reviews documentation and ensures that appropriate documentation is accurate, legible, and submitted by assigned staff within 24 hours of visit. Directs the
dissemination of documentation to the PCP and/or managed medical group at a frequency agreed upon by PCP and/or managed medical group.
5. Coordinates the day-to-day services provided by assigned staff and Medical Professionals to ensure delivery of quality and appropriate care. Organizes, coordinates,
and participates in palliative care team meetings. Supervises, guides and serves as a resource to Skilled Nurses and other supportive personnel.
6. Utilizes the Roze Room information system effectively to enter/update patient and staff information and generate required reports.
7. Participates in satisfying Human Resources requirements, including assuring that staff credentials are supplied to Human Resources in a timely manner.
8. Reports to Vice President of Operations or designee on palliative care team issues, concerns and needs.
9. Initiates resolution process for patient/family grievances, problems, and concerns to ensure that patient/family, PCP and/or managed medical group needs and expectations are met.
10. Demonstrates flexibility in scheduling.
11. Maintains knowledge of state and federal regulations and JCAHO standards.
12. Participates in QAPI program.
13. Observes for safety hazards in the organization’s environments and takes appropriate steps to reduce or eliminate them.
14. Adheres to the highest standards of professional and personal conduct in the performance of his/her duties.
15. Maintains continuing education through academic studies, seminars, and workshops.
16. Attends and participates in in-services, staff meetings, and patient care conferences.
17. Complies with policies and procedures, laws and regulations and standards of practice.
18. Performs other tasks as assigned.
