POSITION OVERVIEW:

The Operations Coordinator is responsible for assisting the Directors with overall operation of the Family Caregiver and Host Home programs. The Operations Coordinator is responsible for training and ongoing support of Family Caregivers and Host Home Providers. The Operations Coordinator is responsible for assisting the Director with the recruitment and selection of host home providers and for ensuring that host home providers and family care givers meet all contractual requirements and support the individual as required by regulation and as defined in the Individual Service Plan. The Operations Coordinator must routinely exercise discretion and independent judgment in the performance of job duties.

JOB FUNCTIONS:

Job functions are considered essential (all individuals in this position must be able to complete these duties with or without reasonable accommodation) unless otherwise denoted in italic type. Duties noted in italic type may be reassigned by Director of Host Homes/Family Care Givers.

1. HOST HOME & FAMILY CARE GIVER RECRUITMENT, SELECTION, AND SUPPORT: Accepts and screens applications for prospective host home providers and Family Care Givers with the Directors. Ensure all applicants and potential independent contractors have all ROOTS and Regulatory trainings and certifications. Ensures that the home meets the needs of the individual, including but not limited to safety and accessibility. Upon identification of a possible placement, schedules visits to the prospective host home for the individual. Ensures that prospective provider documents visits on the designated forms. Reviews visit documentation and discusses visit outcomes with the team (including individual/guardian), allowing the team to make determination about placement.

2. MONITORING: Reviews quarterly monitoring forms for accuracy and timely completion. Conducts periodic unannounced visits in accordance with agency monitoring schedule and as instructed by the Co-Directors. Conducts high level monitoring as assigned by directors. Communicates any concerns noted during monitoring activities with Directors. Assists the Directors with follow-up needed from Host Home Providers and Family Care Givers and follow-up from monitoring visits. Tracks and ensures meeting follow-up takes place in accordance with criteria in IDT/planning meetings.

3. CONTRACTOR MONITORING AND COORDINATION: Review and update contractor spreadsheet to ensure qualifications and training remain in compliance with state regulations and ROOTS Policy. Facilitates annual training documentation process and coordinates completion and maintenance of contractor training records.

4. HUMAN RIGHTS COMMITTEE (HRC) PREPARATION: Complete required monthly paperwork to be submitted to the Case Management Agency, including all corresponding documentation. Ensure that all HRC signoff documentation is maintained in the individual record in accordance with state regulations. Maintain close communication with Directors regarding HRC submission process.

5. RESIDENTIAL BACK-UP: In conjunction with the Directors, assures that host home providers and Family Caregivers comply with requirements related to use of residential back-up services including assuring that host home providers:

· Use qualified back-up providers in accordance with state regulations and Roots Policy.

· Provide notice of use of back-up providers.

o Assists with arranging emergency residential back-up and ensuring qualified providers receive Person Specific Training.

6. DESIGNATED MEDICATION REMINDER BOX TRAINER: Conduct and maintain training records for all providers and/or individuals using a medication reminder box to administer medications. Conduct re-training as needed or as requested by Directors.

7. DIRECTOR SUPPORT:

· Scheduling: Assist in coordination of meetings and/or trainings requiring attendance by Director(s).

· Meetings: Attend meetings with directors, complete and maintain meeting notes. Attend meetings as Director representative as delegated.

8. ON CALL: Provides on-call support as needed to assigned Host Home Providers/Family Care Givers. Notifies Director(s) of all emergencies after hours that rise to the level of that notification. Ensures there is support and communication with Host Home Providers and Family Care Givers to maintain the health and safety of individuals.

9. POLICIES/STANDARDS/MONITORING: Maintains knowledge of ROOTS policies, regulatory requirements and licensing standards and assures program compliance. Participates in surveys by regulatory agencies.

10. INSERVICE: Participates in the development of in-service training modules and facilitates in-service training as needed. Attends required training and maintains required certifications.

11. OTHER DUTIES: Performs other duties as assigned.

12. ADDENDUM: A list of assignment specific duties included as an addendum to this job description. The addendum may also identify assignment specific job requirements/specifications.

JOB REQUIREMENTS/SPECIFICATIONS:

Education/Experience/Skills/Knowledge/Licenses:

· 21 Years of Age

· High School Diploma or GED

· Successful completion of ROOTS training

· Demonstrated competency in CPI, CPR, First Aid and Medication Administration.

· QBS Safety Care or CPI-Must demonstrate competency in personal safety techniques

· Medication Administration-Must achieve and maintain medication administration and QMAP certifications.

· First Aid and CPR-Must achieve and maintain 1st Aid and CPR certification.

· Satisfactory results from CBI screening and reference checks per guidelines

· Valid driver’s license and driving record that meets ROOTS standards

· Two years paid work experience with individuals with disabilities.

· Knowledgeable regarding State of Colorado and Federal regulations regarding HCBS-DD Waiver Services

· Carries cell phone (personal or company) for accessibility when away from the office during work hours.

· Personal vehicle for work-related travel.

Work Environment: Job functions are performed in an environment that promotes individual autonomy and independence. Most job functions are performed in an office or residential environment. Routine visits to individual residences are required. Driving is required on a regular or frequent basis.

Physical Requirements: Ability to visit individual residences. Ability to operate office equipment including, but not limited to, telephone, computer, fax and copy machine.

Financial Accountability: Oversight of individual funds; limited purchasing authority.

Job Type: Full-time

Pay: $24.00 - $32.00 per hour

Expected hours: 40 per week
