POSITION OVERVIEW:

The Host Home/Family Caregiver Monitoring Specialist is responsible for assisting the Directors with overall operation of the Family Caregiver and Host Home programs. The Monitoring Specialist is responsible for training and ongoing support of Family Caregivers and Host Home Providers. The Monitoring Specialist is responsible for assisting the Host Home Director with the recruitment and selection of host home providers and for ensuring that host home providers and family care givers meet all contractual requirements and support the individual as required by regulation and as defined in the Individual Service Plan. The Monitoring Specialist must routinely exercise discretion and independent judgment in the performance of job duties.

JOB FUNCTIONS:

Job functions are considered essential (all individuals in this position must be able to complete these duties with or without reasonable accommodation) unless otherwise denoted in italic type. Duties noted in italic type may be reassigned by Director of Host Homes/Family Care Givers.

1. PROGRAMATTIC: Promotes and fosters a supportive and person-centered atmosphere for individuals residing in host homes and family care giver settings. Supports providers and staff to ensure that individuals have opportunities to exercise choice and pursue personal goals. Ensures that individuals are involved in all aspects of the daily routine in their homes. Assures that individuals receive all services and supports identified in their Service Plans and required by regulation including implementation of Individual Service and Support Plans, Behavior Support Plans, nursing care plans/protocol, therapy programs/protocols and other identified services and supports.

2. MONITORING: Establishes and maintains monitoring systems to ensure that the individual receives all services and supports required by their services plans and by applicable regulations. Ensures that monitoring visits occur in accordance with the individual’s monitoring plan, including periodic unannounced visits as instructed by the Co-Directors. Completes and submits monitoring for assigned caseload of individuals that reside in host homes and family care giver settings. Assists the Directors with follow-up needed from Host Home Providers and Family Care Givers and follow-up from monitoring visits.

3. RESIDENTIAL BACK-UP: In conjunction with the Directors, assures that Host Home providers and Family Caregivers comply with requirements related to use of residential back-up services including assuring that host home providers:

· Use qualified back-up providers in accordance with state regulations and Roots Policy.

· Provide notice of use of back-up providers.

o Assists with arranging emergency residential back-up and ensuring qualified providers receive Person Specific Training.

4. THERAP: Develops secure electronic documentation system within Therap. Ensures all information is input initially, updated as needed and reviewed quarterly. Provides re-training on documentation requirements as needed for Host Home Providers and Family Caregivers.

5. SERVICE SUPPORTS: Monitors for completion and review with provider required Service Supports tasks in compliance with Colorado Rules and Regulations:

· HEALTH & SAFETY ASSESSMENTS: Develops and monitors medication, safety and nutrition plans for individuals receiving Individual Residential Services and Supports. Assures that Health/Safety plans are updated annually. Facilitates quarterly review of Health/Safety plan with the person and his/her host home provider or staff.

· ISSP’s: In accordance with goals established at the SP and in conjunction with the individual and other team members, develops Individual Service and Support Programs (ISSPs); Assures that ISPs comply with agency standards and policies and state regulations. Provides Host Home Providers and Family Care Givers training and maintains training on ISP programs. Monitors individual progress.

6. BEHAVIOR HEALTH SERVICES:

· MONITORING: Assure behavioral data is completed by the Host Home Provider/Family Care Giver at the required frequency. Monitor behavioral data and criteria for completion on BSSP’s. Review and prepare information for psych clinic appointments.

7. POLICIES/STANDARDS/MONITORING: Maintains knowledge of ROOTS policies, regulatory requirements and licensing standards and assures program compliance. Participates in surveys by regulatory agencies.

8. INSERVICE: Participates in the development of in-service training modules and facilitates in-service training as needed. Attends required training and maintains required certifications.

9. OTHER DUTIES: Performs other duties as assigned.

JOB REQUIREMENTS/SPECIFICATIONS:

Education/Experience/Skills/Knowledge/Licenses:

· 21 Years of Age

· High School Diploma or GED

· Successful completion of ROOTS training

· Demonstrated competency in CPI, CPR, First Aid and Medication Administration.

· QBS Safety Care or CPI-Must demonstrate competency in personal safety techniques

· Medication Administration-Must achieve and maintain medication administration and QMAP certifications.

· First Aid and CPR-Must achieve and maintain 1st Aid and CPR certification.

· Satisfactory results from CBI screening and reference checks per guidelines

· Valid driver’s license and driving record that meets ROOTS standards

· Carries cell phone (personal or company) for accessibility when away from the office during work hours.

· Personal vehicle for work-related travel.

Work Environment: Job functions are performed in an environment that promotes individual autonomy and independence. Most job functions are performed in an office or residential environment. Routine visits to individual residences are required. Driving is required on a regular or frequent basis.

Physical Requirements: Ability to visit individual residences. Ability to operate office equipment including, but not limited to, telephone, computer, fax and copy machine.
