Richmond Behavioral Health is seeking a qualified Permanent Supportive Housing Outreach Worker to join our Adult Mental Health Team . The individual will perform intermediate professional work assessing client needs, developing, implementing, and monitoring service delivery and assistance plans, coordinating and monitoring services with other agencies, counseling and assisting clients, maintaining records and files, preparing reports, and related work as apparent or assigned. Work is performed under the moderate supervision of the assigned supervisor.
Essential Functions
Serve as liaison between RBHA Adult Mental Health case managers and PSH program regarding PSH referrals and pre-housing work. Works with Adult Mental Health Case Managers to identify potential referrals and provides support in completing PSH applications.
Facilitate PSH approval process and all necessary related processes and tracking from application to housing.
Facilitates warm handoffs from outreach (homelessness) to housing specialist (housed).
In coordination with RBHA PATH funded outreach, provides outreach and support at shelters, lunch sites, places not meant for human habitation (for example, streets, woods, under bridges) and other community locations to individuals who have been identified as appropriate PSH referrals. Approximately 50% of outreach services are provided directly to individuals in the community and at offices of agency partner providers.
Assesses individual needs, capabilities, and appropriateness for services; presents options and services based upon needs assessment.
Makes referrals and linkages to appropriate agencies for services, coordinates services and treatment with multiple service providers and agencies.
Evaluates individual's environment for safety, security, negative factors, and productivity.
Assists with orienting and training newly hired staff.
Serves as liaison to public agencies and provides information regarding RBHA programs and services.
Attends inter-disciplinary team meetings to discuss decisions for plan of care; provides or arranges transportation.
Prepares a variety of reports; prepares and maintains charts and other records.
Attends meetings, staffings, and conferences as they relate to program needs.
Assists in curriculum development and resource tools.
Enters appropriate data into community wide system (Homeward Community Information System - HCIS) in compliance with applicable regulations.
Meets on a regular basis with community coordinated outreach providers, to coordinate care, support individuals with high needs, and discuss service gaps and needs.
Position Requirements

Knowledge, Skills, and Abilities
Knowledge and understanding of racial disparities and the impact on those experiencing homelessness; General knowledge of community and agency resources and programs for the community service population as well as those experiencing homelessness; general knowledge of interviewing and supportive counseling techniques including motivational interviewing and progressive engagement; basic understanding of trauma informed care; general knowledge of social casework techniques; general knowledge of human development and behavior; ability to solve problems within scope of responsibility; ability to analyze facts and exercise sound judgment in arriving at conclusions; ability to communicate complex ideas effectively, orally, and in writing; ability to prepare clear and concise reports; ability to establish and maintain effective working relationships with individuals, medical professionals, community partners, agencies, associates, and the general public; Knowledge of different housing interventions, including Permanent Supportive Housing.
Education and Experience
Bachelor's degree with coursework in social work, psychology, counseling, sociology, health services, or related field strongly preferred, or equivalent combination of experience that would satisfy the requirements of this position. Moderate experience working with individuals with serious mental illness preferred. At least one year working with individuals experiencing homelessness required.
Qualifying staff will be eligible for a $3K sign-on bonus as defined by the terms and conditions of the approved sign-on bonus policy.

Starting Salary Range
$21.79/ hr
