Rhode Island Hospital is Hiring a Transfer and Access Ctr Coord Near Providence, RI

Summary:
Under the direct supervision of the Clinical Manager (or designee) the Transfer & Access Center Coordinator will communicate with physicians and staff at referring institutions and RIH/Hasbro Childrens hospital in coordinating critical care patient transfers. Individual will serve as the clinical and administrative liaison for said transfers in accordance with established departmental policies and procedures.

Responsibilities:
Requests and/or provides information regarding the transport of ill or injured patients to and

from RIH and Hasbro Childrens Hospital. Serves as coordinator for the admissions and transfers of said patients.

Provides monitored communication links among persons necessary for consultation referral and transport of patients utilizing state of the art communications equipment.

Maintains communication with transport team and tracks estimated times of arrival and departures.

Completes all required documentation with emphasis on clarity and completeness within appropriate time frame.

Coordinates demographic and insurance company information with Admitting Department.

Maintains specialized logs and databases related to transports. Collects statistical data on patient numbers types and demographic information.

Reviews at start of shift bed availability with bed control and transport team. Coordinates appropriate and efficient placement of patients guided by protocols and working with the

Admitting Department and clinical units.

Appropriately directs information to the Managers and Medical Directors.

Assures that all communication and support equipment is properly functioning.

Assures that department is properly stocked with office supplies and paperwork.

Participates in community outreach and in-house educational efforts such as nursing grand rounds etc.

Follows up with referring facilities regarding patient outcomes and transport-related issues in conjuction with transport staff.

Meets and maintains mandatory/safety education and clinical competencies measured by pre-determined credentialing standards.

Maintains collaborative relationships with team members other members of the health care system and outside agencies including but not limited to: transferring and receiving hospital customers EMS personnel and community agency representatives.

Uses effective interpersonal skills to identify and resolve conflicts decrease communication barriers and facilitate group effectiveness.

Participates in Quality Assurance both in meetings organized for that purpose and on a daily basis using established quality indicators for department.

Participates in the orientation of new employees as assigned.

Maintains patient confidentiality and dignity. Complies with all HIPPA regulations.
Other information:

Licensure as Registered Nurse in the State of Rhode Island by the Rhode Island Board of Nursing or licensure as a Registered Nurse in accordance with the Nurse Licensure Compact agreement of the National Council of State Boards of Nursing.

QUALIFICATIONS:

Must have current licensure to practice as a Registered Nurse in the State of Rhode Island.

Baccalaureate in nursing preferred.

Must complete credentialing specific to Transfer & Access center within 3 months.

Three to Five years current experience as a professional nurse providing care within an intensive care unit or emergency department is required.

Critical Care Transport experience preferred.

Strong computer skills with knowledge of computer programs such as excel outlook PowerPoint access and word which will be used for retrieval of demographic information performance data and statistics.

Ability to operate transmission equipment.

Highly effective verbal and written communication skills are required to work with transport staff physicians and other hospital personnel and patients.

Ability to demonstrate effective customer service behaviors.

WORK ENVIRONMENT AND PHYSICAL REQUIREMENTS:
Visual and aural ability to work with computer and transmission equipment.

Physical requirements include the ability to sit for long periods.
