Overview:

As a patient care coordinator, you will be responsible for coordinating and managing the care of patients with chronic conditions to optimize their health outcomes and quality of life. You will work closely with patients, their families, healthcare providers, and other members of the healthcare team to develop and implement personalized care plans.

Responsibilities:

• Patient Assessment and Care Planning:
• Conduct comprehensive assessments of patients with chronic conditions to identify their healthcare needs, goals, and preferences.
• Collaborate with patients and their families to develop individualized care plans based on evidence-based guidelines and best practices.
• Continuously evaluate and update care plans in response to changes in patients' health status or needs.
• Care Coordination and Communication:
• Serve as the primary point of contact for patients and coordinate their care across multiple healthcare settings, including hospitals, clinics, and community resources.
• Facilitate communication and collaboration among members of the healthcare team, including physicians, specialists, pharmacists, and other providers.
• Educate patients and their families about their chronic conditions, treatment options, medications, and self-management strategies.
• Health Monitoring and Support:
• Monitor patients' health status and adherence to treatment plans through regular check-ins, telehealth visits, and remote monitoring technologies.
• Provide ongoing support and coaching to help patients manage their chronic conditions, achieve their health goals, and improve their self-care skills.
• Identify and address barriers to adherence and self-management, such as medication side effects, financial concerns, and social determinants of health.
• Documentation and Reporting:
• Maintain accurate and up-to-date documentation of patient encounters, assessments, care plans, and interventions in electronic health records (EHRs) or care management platforms.
• Generate reports and data analytics to track patients' progress, outcomes, and satisfaction with care.
• Ensure compliance with regulatory requirements, billing codes, and documentation standards for chronic care management services.
• Other:
• Provide education to caregivers on medication administration and CPR.
• Administer vaccinations to patients.

Qualifications:

• Licensed Practical Nurse (LPN) or Registered Nurse (RN) licensure in Missouri.
• Experience in care coordination, case management, or chronic disease management.
• Strong clinical assessment, critical thinking, and problem-solving skills.
• Excellent communication, interpersonal, and patient engagement skills.
• Proficiency in using electronic health records (EHRs) and telehealth technologies.
• Commitment to providing patient-centered, evidence-based care and promoting health equity.

Working Conditions:

• The work setting will be varied and flexible. You will work from the office, home, and also visit patients in their homes or community settings.
• Will require occasional travel within the local area to attend meetings, trainings, or patient visits.
• Typically works regular business hours with flexibility.

Job Types: Full-time, Part-time

Pay: $20.00 - $30.00 per hour
