Job Description
Job Summary
The Case Management RN practices within the guidelines of the Nurse Practice Act and is responsible for assessing, planning and coordinating patient care in a high quality, cost effective manner
Essential Functions
  • Assists physicians and hospital staff in appropriate utilization of resources through application of utilization criteria and facilitating timely discharge planning for patients
  • Coordinates services between hospital departments to facilitate timely patient discharge
  • Conducts concurrent review of patient records on admission to the hospital and as needed
  • Applies utilization criteria accurately in order to determine appropriate utilization of resources
  • Notifies designated internal and external contacts of utilization issues that may affect patient care and/or reimbursement
  • Acts as a resource/advisor to physicians regarding discharge planning activities and all issues that may affect resource utilization and reimbursement
  • Maintains knowledge and understanding of CMS regulations, Medicare, managed care, and other payer regulations and benefit limits
  • Develops and maintains knowledge and understanding of hospital and community resources, and facilitates use of most appropriate level of care to conserve patient, hospital, and payer resources
  • Participates in collecting and recording data for utilization, outcomes, and process improvement reporting
  • Works collaboratively with patients, family members, physicians, hospital staff, and other individuals and agencies involved in providing care
  • Communicates in a professional and positive manner in all interactions with internal and external customers
  • Prioritizes and organizes daily work assignments in a timely and efficient manner
  • Maintains promptness of arrival to work
  • Adheres to absenteeism according to hospital and departmental policy
  • Apply utilization criteria accurately
  • Collaborate with physicians and other health care providers
  • Any other duties as needed to drive to the Vision, fulfill the Mission, and abide by the Values of the organization
Qualifications
  • Associate's Degree Nursing required
  • Bachelor's Degree Nursing preferred
  • Minimum of 2 years Nursing acute care clinical experience or acute care utilization review/case management experience required
  • Minimum of 1 year Utilization Review, Case Management and Orthopedic experience preferred
  • RN - Registered Nurse - State Licensure And/Or Compact State Licensure In the State of Missouri Upon Hire required
