JOB SUMMARY

A professional who provides skilled nursing services within the scope of nursing practice standards in collaboration with primary care providers; Directs and guides patient care, teaches and educates patients and their families. Professional skilled nursing is performed under the supervision of the Administrator/Director of Clinical Services. Maintains and delivers the highest quality customer service standards

RESPONSIBILITIES/ESSENTIAL FUNCTIONS

• Demonstrates proficiency with nursing skills in preventative and rehab nursing procedures
• Performs socio-psychological and physical assessments along with home safety/environmental assessments, discharge plans.
• Documents per agency guidelines and in the required 24 hour time limit.
• Develops Individualized Plan Of Care to be submitted to the physician for approval and implementation and performs ongoing revisions as needed.
• Performs physical examination and review of all body systems and documents accordingly.
• Communicates with the patient's medical providers, community partners, Clinical Manager, Relief team to coordinate effective treatment modalities and goals
• Participates in: the Performance Improvement Committee process as requested; required monthly staff meetings; and assisting in the planning, operation and evaluation of the nursing services of the organization.
• Reviews billing processes with patient and/or family advising patient/family when co-pay or Medicare is not likely to pay for services.

SPECIAL EQUIPMENT TO BE OPERATED

Vehicle to do routine patient visits, possible transfer/lifting devices, medical supplies

WORK ENVIRONMENT

Indoors, performing patient care assessment/tasks in patient’s residence

Travel to patient residences/meetings; will require transportation to get to various work sites

Possible exposure to communicable diseases

QUALIFICATIONS/REQUIREMENT

Graduate of RN Nursing Program;

Currently licensed as a Registered Nurse in the Commonwealth of Massachusetts

Minimum of one year of experience as a professional nurse, preferably in home care.

Must have a criminal background check.

Must have current CPR certification.

Must read, write and communicate in English
