Woodlake at Tolland is a short and long term care facility located in Tolland, CT. We are currently seeking a Care Plan Coordinator to join our team!

This is a full time Monday-Friday, salaried position and requires shared weekend on call hours.

PCC and Care Planning knowledge is a must. Previous experience in a SNF is strongly preferred.

RN required. Candidates must possess a valid CT RN license in good standing.
