Summary: The incumbent in this position is responsible for providing community and client centered counseling, education and information concerning illnesses, maternal child health, well-being and chronic disease management.
Duties and Scope of Responsibilities: Visit clients in their homes or community sites and make appropriate telephonic contact to assess, plan, and implement a plan of care, evaluate client's response to plan, make changes in plan as appropriate
•Identify participant's risk factors, concerns, priorities, strengths and resources on an on-going basis and track receipt of all services
•Performs psychosocial and nursing assessments for prenatal and infant clients.
•Provide health related counseling on the prevention of communicable diseases; high risk conditions of pregnancy, neonates, infants and children; coordinate health programs with outside organizations and agencies; introduce general health related information and instruction to individuals, families and community groups.
•Provide education and information on a variety of pre-examination tests including skin tests, blood tests, and other tests used in determining communicable disease before and after tests as necessary.
•Assess and evaluate client's response to treatment and medication.
•Act as a nursing liaison between clients, MCO, physicians, provider offices, community partners and government agencies ensuring client has access to quality care
•Conduct screening to identify risk factors such as domestic violence, substance use/abuse, perinatal maternal distress and/or mental health issues
•Screen participant for anxiety, depression and Adverse Childhood Experience (ACE)
•Provide education to participants utilizing program materials to reinforce care plan regimen implemented by medical care team, promotion of medical compliance and progress on medical goals
•Assist participants with accessing medical care and monitor the effectiveness of services and adjust care plan as needed
•Ensure that each participant schedules and completes postpartum visit and provide postpartum education authorization from the participant
•Collect and enter data into the designated data base and participate in quality improvement activities as directed
•Provide disease and chronic illness management education, referral and follow-up services, including referrals to subspecialists, and other needed services.
•Provide instruction and education to help participants self-manage chronic, pregnancy-induced or pregnancy related medical conditions.
•Provide information on medication, side effects, risks of noncompliance
•Conduct necessary follow up calls with participants to obtain any needed information, remind them of appointments and provide additional education
•Complete all necessary programmatic and organization forms, documents and reports in a timely manner
•Participate in proactive team efforts to achieve departmental and company goals
•Provide leadership to others through example and sharing of knowledge/skill
•Follow all organization's policies, practices, and procedures
•Chart and document home visits in a timely manner and evaluate referrals
•Perform other duties as assigned
Special Skills/Licenses/Training/Certifications Necessary:
•Ability to communicate with a range of constituents, prioritize work effectively in a fast-paced, deadline-oriented office environment.
•Excellent interpersonal and writing skills
•Reliable transportation with proof of valid driver's license and auto insurance or the ability and means to travel on a flexible schedule for conducting home visits and other required community-wide travel.
•Bilingual (English/Spanish) required for some positions
Education/Training/Experience:
•Bachelor's degree in Nursing (BSN)
•Valid Florida RN License required.
Qualification/Requirements:
•1-3 years in maternal and child heath, behavioral health nursing, pediatrics, public health or other fields.
•Community/Public Health Nursing experience including experience with home visits preferred.
•Proficient personal computer experience to include Microsoft Outlook, Word, Microsoft Access, Excel, PowerPoint and Internet applications
