Job Summary

To provide case management services, direct services and advocacy for youth involved in the Alternative to Corrections through Education (ACE) program. Develop and maintain relationship with court and legal system, document casework activities, and provide evaluation, advocacy and follow-up to youth and their families throughout their court involvement or during the terms of their informal dispositions. Run specified groups, conduct individualized behavioral meetings, act as an intermediary with youth and Juvenile Court, in secure detention.

Essential Duties

  • Review and analyze information relating to child or caregiver's social, psychiatric, and medical history to learn the nature of the functional limitations as well as strengths each child and family member present.
  • Obtain and assess relevant information regarding child's development, i.e., social, familial, medical, and vocational strengths.
  • Conduct family and individual risk and needs assessments as well as continually assess the safety of youth on the caseload.
  • Run specified groups with the ACE youth and provide a role model and mentor for youth.
  • Provide youth and family with appropriate information regarding services offered by the Human Services Department or the community.
  • Conduct a thorough assessment of youth's strengths, needs, and individual and family resources. Conduct home visits with parents/caregivers to discuss youth process and assess the parent/caregivers change process on a monthly basis.
  • Monitor the youth's compliance with the ACE program; review and revise plans as necessary.
  • Refer to and coordinate the various services to implement the treatment plan.
  • Utilize the Youth Assessment Screening Instrument (YASI) in planning for youth.
  • Produce accurate and timely documentation and reports including but not limited to assessments, treatments plans, progress reports and court reports in order to insure compliance with federal and state requirements; maintain necessary e-WISACWIS documentation supporting these activities.
  • Recommend to management changes or gaps in the system that prevent the delivery of services.
  • Prioritize and organize time to cover assigned area and manage caseload in equitable manner.
  • Maintain knowledge of pertinent departmental, state and federal policies, rules and regulations including state statutes 48 and 938, Access and Ongoing standards.
  • Consult with other professionals in the field and in related fields to increase individual knowledge and understanding of various disciplines, changes and developments in the field of Human Services.

Supervision Received

Receives general supervision from the Youth Programming Supervisor.

Qualifications

  • Bachelor's degree in a Human Services field required (Child development, criminal justice, educational psychology, vocational rehabilitation, guidance and counseling, psychology, social welfare, social work, or sociology).
  • One (1) year full time post-graduate experience as a Social Worker or in a related professional position such as providing child protection services to children, participating in child or adult protection investigations, case managing youth under CHIPS, JIPS or Juvenile Court Orders, participating in or supervising treatment programming for children or juveniles involved in the child welfare or juvenile justice systems, or providing services to children and families through a child placement agency.
  • Social work experience must include experience in at least 4 of the following activities: psycho-social assessment, treatment plan development, treatment plan implementation, use of community resources, providing direct services, conducting child or elder abuse/neglect investigations, development of social histories, or initiating court actions including acting as petitioner in court.
  • Valid Wisconsin driver's license for required travel. Personal automobile, if used for work-related travel, must be covered by an automobile insurance policy.
  • Prompt and regular attendance.
  • Or any equivalent combination, training and experience, which provides the requisite knowledge, skills and abilities.

Knowledge Skills & Abilities

  • Experience using WISACWIS preferred.
  • Knowledge of Wisconsin State Statutes Chapter.
  • Knowledge of fundamental social work principles.
  • Knowledge of community resources.
  • Ability to deliver concise oral and written reports.
  • Ability to work well with other agencies.
  • Ability to respond to crises and to make appropriate judgments as to resolution.
