General Statement
The Office of Counseling and Wellness at Quinsigamond Community College fosters an integrated model of holistic wellness that addresses the needs of a diverse student population through the provision of healthcare services that address the intellectual, physical, emotional and social needs of students. Many times, the students require support or resources that require help in accessing them in the community.
The Case Manager provides support resources by coordinating student referrals with clinical and non-clinical student consultation and maintaining records of student cases.
This position works closely with other departments and entities on campus, including but not limited to the Dean of Students, CARE Team, Student Accessibility Services, and the Dean of Compliance to track students with specific challenges and ensure a successful transition to and retention in the college environment.
The majority of the Case Manager's time is allotted to maintain and monitor a system for continuity of care for clients. Case management may include meeting with students to support a transition process from intensive outpatient or inpatient mental health services, hospital visits, obtaining external referral services, maintaining database of community providers, and coordination of care, including follow up communication and/or meetings with students or community providers. This position requires excellence in service delivery and organization.
Duties and Responsibilities
Under the supervision of the Director of Counseling and Wellness
1.Serve as a referral coordinator for students in need of social service and mental health resources off campus.
2.Coordinate student transitions from QCC services to external, community providers if necessary (e.g., psychiatric/psychological treatment services, alcohol/drug/addiction treatment services, return to parental care). Re-evaluates and adjusts the plan according to need over time; partner with the student and collaborate with other relevant parties.
3. Communicate with staff, parents, families, or designated emergency contacts regarding emergencies including hospitalizations and other crises to help facilitate success on campus, or transition back to campus when issues are appropriately resolved.
4.Serve as a resource and case manager to QCC students with specific challenges in order to support academic and personal success.
5.Provide support for students in crisis as appropriate.
6.Complete records punctually according to record keeping/report writing procedures that are currently operative at the Counseling and Wellness Center and required by legal and ethical regulations.
7. Utilize diversity and social justice practices to promote student success as it relates to core duties.
8. Provide support for walk in hours (brief assessment and assign to ongoing counselor).
9.Represent QCC Counseling and Wellness Center in Community Based Task Force/Committees as needed.
10.Participate in evening and weekend activities and programs.
11.Other duties as assigned.
Minimum Qualifications
1.Bachelor's degree in Human Services or Social Work
2.Ability to provide brief and focused assessment of community needs
3.Knowledge of community-based resources
4.Ability to collaborate with other social service agencies
5.Demonstrated competency working with multicultural and other diverse populations.
6.Experience working independently and collaboratively in a fast paced work setting.
7.Experience and skills with the use of technology to enhance services.
