Queens Nassau Rehabilitation and Nursing Center is Hiring a Mds assessor Near Queens, NY

Reports to Director of Nursing and Administrator

Summary : The primary purpose of the MDS Assessor is to continuously monitor, evaluate, and manage the care given to residents.

Responsibilities include preparing and electronically transmitting timely reports to the national Medicare and Medicaid database.

You will also be responsible for ensuring the accuracy of medical records and coding procedures which are essential to receive proper reimbursement.

In this role, you will perform care area assessments and care planning process plans, and ensure that our residents' wellbeing and health are being monitored and recorded.

Qualifications :

1. An active NYS Nursing license.

2. A solid understanding of data collection, reporting, and analysis.

3. The ability to draft and create thorough care plans for residents, unless limited by licensing.

4. Exceptional communication and analytical skills.

5. An excellent work ethic and understanding of MDS procedures.

6. Superb organizational and interpersonal skills.

7. One year of long-term care clinical nursing experience and one year of clinical supervisory, management or resource experience preferred.

8. Experience / expertise in gerontological nursing is necessary to perform the essential functions of this position.

Essential Functions :

Responsible for, but not limited to :

  • Schedules Care Conference Reviews
  • o with each admission;
  • o quarterly;
  • o annually;

o and if there if there are significant changes.

  • Generates a weekly list of Care Conference Reviews and sends to the team.
  • Weekly and / or daily review of the resident’s status is completed with the interdisciplinary team as needed to determine skilled need continues and to evaluate if other MDS are needed.
  • Transmits data, per CMS regulations, after facility completes a resident assessment.
  • Responsible for getting certifications signed by the MD per the guidelines set forth by CMS.
  • Responsible for completing certifications for Managed Medicare Residents.
  • Responsible to close the care plan upon the resident discharge and set an appropriate discharge MDS.
  • Completes and submits discharge MDS’ timely.
  • Update admission names and discharges on the 672 each week.
  • Seeks continuing education offered through Relias, webinars, articles etc.
  • Expected to be at daily morning meeting and Utilization Review weekly meeting
  • When extreme situations arise- maybe assigned floor duties.
