Compensation Details
  • Up to $257,000 - $300,000 total annual compensation (salary bonus)
  • $240,000 - $260,000 annual base salary, dependent upon experience
  • 15% annual target bonus, based upon performance. Bonuses will be achievable for all clinicians

About the role:

Quartet medical directors serve a key role in helping lead our collaborative, multidisciplinary care teams in our Whole Health program. This program is focused on patients with serious mental illness (SMI), and our program brings a collaborative approach to meeting each patient's medical, behavioral, and social needs. The teams include advanced practice clinicians, therapists, care coordinators, social workers, peers, psychiatrists, network managers, and case managers. Together this team is responsible for providing coordinated, patient-centered care for an intimate panel of patients.

As a Quartet medical director, you will co-manage and provide clinical supervision for a longitudinal panel of patients with advanced practice clinicians. You may also independently carry a small panel of high-risk, complex patients. You will remove barriers to accessing care and bring care to our patients seeing them in the home, community, and virtually. In collaboration with our APCs, you will ensure comprehensive holistic care, chronic disease management, excellent preventative care and wellness, and prompt urgent care as needed. Our smaller panel sizes, paired with robust support from a multi-disciplinary team, means that you will be able to spend more time with patients and have a greater impact on their health and well-being.

We expect up to 50% of your time may be spent providing direct clinical care, with the rest of your time will be spent in a clinical supervisory and management capacity (e.g. case conferences, external provider communication, etc).


Clinical responsibilities will include:

  • Provide oversight of the care team, inclusive of advanced practice clinicians, to ensure exceptional, evidence-based, and patient-centered care
  • Serve as the primary clinician for a small panel of high risk, complex patients Perform home and community-based follow-up and urgent care visits, as well as virtual visits in the care of your co-managed patients
  • When needed, performing annual wellness visits and patient assessments to obtain a comprehensive biopsychosocial view of patients' health, which includes addressing quality gaps in care
  • Facilitating multidisciplinary case rounds, which includes guiding the teams to create appropriate patient-centered care plan that address root drivers of health and ensure execution of the care plan
  • Collaborate with external providers and community-based organizations to advocate for patient care aligned with their goals
  • Provide clinical coverage as needed, including performing home-based follow-up and urgent visits when appropriate
  • Participate in weekend and off-hours on-call schedule to address patients' urgent clinical needs
  • Ensure timely and appropriate documentation within our electronic medical record and systems that support care for our patients

Managerial responsibilities will include:

    • Ensuring timely, accurate, and appropriate documentation for APC visits and visits by other clinicians
    • Ensuring quality gap closure for your co-managed panel of patients.
    • Performing 1:1 in-person observations as well as case and chart reviews on a regular basis
    • Coaching and training advanced practice clinicians in clinical care, workflows, EHR documentation, and multidisciplinary team collaboration
    • Participating in workflow, policies & procedures improvement and implementation
    • Participating in interviewing, hiring, and training of clinicians and members of the care team
    • Be a source of inspiration, driving culture and articulating the impact of our program on improving the lives of our patients

Desired skills and experience:

Required

  • Medical Doctor (M.D.) or Doctor of Osteopathy (D.O.) Graduate
  • Active Internal Medicine or Family Medicine Board Certification
  • Active medical license and DEA Controlled Substance Registration in New Jersey. X-waiver licensure (for buprenorphine prescribing) is a plus but not required
  • Certified in Basic Life Support for Healthcare Providers
  • 3 years of clinical experience in providing primary care in an internal medicine, family medicine, or geriatric medicine setting
  • Comfort and experience in working within an electronic medical record
  • Valid unrestricted driver's license and access to an insured vehicle for daily use

Preferred

  • Reside in the greater New Jersey area (which may including bordering cities), with knowledge of the New Jersey market and willingness to travel across the state
  • Experience within value-based care models
  • Fellowship trained in Geriatrics, Addiction Medicine, Palliative Care, are welcome but not required
  • Experience and passion for caring for individuals with complex chronic medical, behavioral and social needs in a multidisciplinary, integrated, or collaborative model
  • Experience and comfort engaging individuals with behavioral health conditions, inclusive of those with serious mental illness
  • Experience with co-managing patients with advanced practice clinicians and with home-based models of care
  • Passion to provide high quality care to all patients using an evidenced based approach
  • Energized by the idea of being part of a new, innovative model of care, and enjoys continuously learning and adapting workflows and processes to improve care
  • Excellent communication skills, and ability to work collaboratively to coach and mentor clinical and non-clinical members of the care team
