Qualifications and Requirements:

• Valid driver's license and proof of car insurance
• Must be a licensed Occupational Therapist Assistant by the state
• Maintains timely and accurate patient records and reports
• Must have at least one-year clinical experience
• Must be able to travel to prospective patient’s place of residence
