Puget Sound Home Health and Hospice is seeking experienced and caring full-time Hospice and Home Health Registered Nurse Case Managers (RN) in the South King County, Washington area!

Essential Duties
- Completes comprehensive in-home patient assessment
- Oversees comfort care and pain management
- Develops plan of care under primary care provider orders
- Provides direct patient care in accordance with the plan of care and scope of practice
- Provides accurate & timely documentation consistent with plan of care
- Participates in QAPI program to assure highest level of patient care
- Participates in interdisciplinary coordination of home health and hospice services
Requirements and Qualifications
- Registered Nurse (RN) licensure
- Prior Home Health and/or Hospice experience preferred
- Ability to work independently and demonstrate critical thinking
- Current BLS certification from AHA
- Be a licensed driver with an insured automobile in good working order
