LEAD IN A DYNAMIC AND GROWING HEALTH CENTER NETWORK SERVING ACROSS THE CITY!

The PHMC Health Network Medical Director will oversee strategic clinical leadership, daily clinical operations and coordination of the primary medical care services across the PHMC Health Network portfolio of federally qualified health centers, medical respites and shelter-based nursing care while providing direct patient care on a part-time basis.

Actively contribute to our powerful mission to be the premier regional provider of integrated, community-based healthcare by combining evidence-based clinical practices, outstanding patient service, innovative care partnerships, and team-driven excellence, within a healthy fiscal environment. Lead with vision and manage relationally in this full-time role to maximize quality, effectiveness, efficiency, productivity and growth in our diverse primary care context. This position reports to the PHMC Health Network Managing Director.

ESSENTIAL DUTIES AND RESPONSIBILITIES

§ Supervise, mentor and support the medical staff to ensure the provision of quality medical care to PHMC Health Network patients.

§ Supervise the Director of Nursing and primary care clinicians to ensure quality outcomes for the team.

§ Guide the patient care teams in achieving a team-based model of care and effective care coordination and care transitions, as within a Patient-Centered Medical Home model.

§ Direct the medical care and health services of the organization to ensure compliance with legal standards and established policies and protocols in the pursuit of target outcomes.

§ Establish, review and authorize medical protocols and standards; monitor medical trends and best practices and present recommendations for modifications and growth.

§ Develop and administer quality assurance and improvement programs, including chart audits, data review and other peer review methods.

§ Enter into collaborative practice/prescriptive authority agreements and serve as the named supervising physician for designated advanced practice clinicians.

§ Develop and review health care plans and review of program requirements, including EHR utilization.

§ Be involved in all initiatives, concerns, and retention involving patient safety, quality, infection control, and risk management and patient service.

§ Maintain confidentiality of clinical records in accordance with HIPAA regulations

§ Responsible for all credentialing files of providers that are requesting and granted privileges; maintain strong understanding of the NPDB, OIG, and other reports to ensure health center staff are qualified and do not exhibit key factors that would exclude them from practicing within the state.

§ Conduct quality improvement conferences, record audits and data management activities.

§ Assist in data collection, program evaluation and clinical assessments of services.

§ Maintain affiliation with professional groups, medical associations, and medical organizations, and represent the FQHC in meetings of these associations and organizations.

§ Advance FQHC and role of advanced practice nurses in medically underserved areas through advocacy and other professional activities.

§ Maintain relationships with managed care, commercial providers, funders and community organizations.

§ Support 340B pharmacy program development and implementation efforts with other program leadership.

§ Lead grants management efforts as needed, particularly in HIV care and other clinical matters.

§ Provide direct patient care to breadth of health network patients within scope of practice, with particular emphasis on patients living with HIV/AIDS, patients receiving medication-assisted treatment and other areas of significant health disparities.

§ Maintain or acquire HIV Specialist (AAHIVS) certification through the American Academy of HIV Medicine

§ Comply with all governing standards and policies promulgated by PHMC, DOH, Joint Commission and other accreditation/regulatory bodies as well as Infection Control and Environment of Care Guidelines and Procedures.

§ Other assignments as PHMC Health Network grows and develops.

Success in this role means that the Medical Director intrinsically and regularly demonstrates cultural competence, excellent interpersonal (including friendliness, and empathy), communication, and leadership skills along with extensive knowledge of current health care needs and issues of children, adolescents, and adults. The Medical Director maintains a special interest in the area of community medicine and population health. They must also collect adequate client data, conduct a physical exam, interpret physical findings, and manage or refer patients as conditions or protocols dictate.

POSITION REQUIREMENTS

EXPERIENCE

§ 5 years clinical experience required.

§ Experience in, ambulatory primary care, community health and/or behavioral health required.

§ Progressive supervisory and or leadership experience required.

§ AAHIVM HIV Specialist certification (or expectation to pursue).

§ Medication-assisted treatment experience.

EDUCATION

§ Graduation from a college or university accredited by the AMA with a Doctor of Medicine degree or accredited by the AOA with a Doctor of Osteopathy degree.

§ Successful completion of an approved program of residency training in Family Practice or Internal Medicine.

§ License to practice medicine in the State of Pennsylvania plus current DEA registration.

§ At least five (5) years of healthcare related experience as a primary care physician, preferably in an outpatient clinical setting, community health center, or Federally Qualified Health Center (FQHC)

§ Familiarity with an electronic medical record and practice management system

PHYSICAL DEMANDS

The physical demands described here are representative of those that must be met by the Medical Director to successfully perform the essential functions of this job. While performing the duties of this job, they will frequently stand; walk; sit; use hands to finger, handle, or feel objects, tools or equipment; reach with hands and arms; balance; talk or hear. The Medical Director will occasionally climb stairs; stoop; kneel; crouch or crawl; taste or smell. They must occasionally lift and/or move up to 25 pounds and operate related office equipment and use necessary tools. Specific vision abilities required by the position include frequent reading and close vision; distance vision; color vision; peripheral vision; depth perception; and the ability to adjust focus.
