Social Work Case Manager

Description

Provide a full range of social work services including, but not limited to, psychosocial assessment, treatment planning, therapeutic interventions, discharge planning, crisis intervention, and resource referral. Partner with the patient and their support system, as well as interdisciplinary teams, both internal and external to the organization, to improve patient care through the effective utilization and monitoring of health care resources. Facilitate the achievement of patient goals and desired clinical, financial, and resource outcomes.

Required qualification:

• Master’s degree in Social Work.
• 1 year of experience as Social Worker or similar occupation.
• Washington State Licensed Social Work Associate – Independent Clinical (LSWAIC) or Washington State Independent Clinical Social Worker (LICSW).
• 1 year of experience in the areas of acute medical or mental healthcare in an inpatient or outpatient setting.

Swedish is the largest not-for-profit health care system in the greater Seattle area.
