  • Awake overnights: Saturdays 8p-9a, Sundays 9p-9a and Mondays 9p-9a (37 hours in 3 days and 4 days off!)

ABOUT THE ORGANIZATION
Providence Support Services, Inc. has been providing services to adults who have intellectual, developmental and/or physical disabilities on Medicaid funded waiver programs since November 2006. PSSI employs staff to work in the Community Living Services Supported Living program and Community Integration Services Customized Community Supports program in Bernalillo county. Community integration & interaction are key parts of the programs that are strongly promoted. 
PSSI's employees advocate for the individuals they support to raise community awareness & lessen stereotyping of people with disabilities. Employees working in all programs will assist individuals with different levels of abilities in developing the life skills necessary to live their lives to the fullest and make the most out of everyday. Employees assist each individual with daily living skills, medication, cooking, cleaning, grooming/hygiene, medical appointments, health, safety, community activities, transportation, etc. 
Experience is not required to work at PSSI in entry level positions. We hire many people that have never worked in the field before. Employees receive training to support adults who have intellectual, developmental and/or physical disabilities before working with individuals and throughout the course of their employment.
Summary of Duties (Essential functions):
The Awake Overnight Counselor position is safety sensitive, hourly and non-exempt position. The Awake Overnight Counselor is responsible for maintaining the security of the home and program participants who reside within that home during late night/morning hours. The Awake Overnight Counselor is the employee who implements the Individual Service Plan (ISP), Behavior Support Plan of the person receiving supports and services under the direction of the House Lead. The Awake Overnight Counselor must stay awake during the overnight shift and complete hourly checks, or more frequent checks if an individual plan or the company states so. The Awake Overnight Counselor is responsible for the health and safety of each program participant, helping each individual learn and develop new skills, providing behavioral support on a daily basis and in times of crisis, creating an environment that provides a life style based on individual and community values, increasing independence and inclusion while developing valued roles in the community and providing transportation to the program participants. The Awake Overnight Counselor is an essential employee who works in a long-term care facility and is also responsible for the additional duties as listed below and/or assigned:
Primary Responsibilities/Essential Job Functions and Duties: Agency Policies and Procedures
  • Abides by the Employee Conduct Policy, Code of Ethics and all other policies and procedures.
  • Exemplifies agency Mission.
  • Works schedule as assigned.
  • Maintains good attendance and punctuality;
Individual Support for Program Participants:
  • Remains awake during the overnight hours and provides overnight sleep monitoring, completes hourly visual checks on program participants, unless a plan notes differently;
  • Must check on program participants (visually see each participant) and call in to the Steadycare system every hour;
  • Is a member of the IDT and works with the individuals’ IDT’s and relates positively to Individuals Supported and their teams;
  • Provides direct support to program participants and works with IDT’s as a participating member of the IDT;
  • Assists program participants with their programs, therapies and plans;
  • Provides assistance, support and training on daily living skills, including but not limited to cooking, cleaning, appropriate manners and behavior, personal hygiene regimen, etc.;
  • Facilitates community activity and inclusion as assigned;
  • Attains training on, knows and implements the program participant’s programs and plans;
  • Observes and implements safety in all facilities, monitors/assures health and safety of program participants in the residence and in the community, including but not limited to safeguarding participants from falls;
  • Assists with medical appointments as needed;
  • Assists with menu planning, follows menus and prepares meals for program participants: must prepare breakfast and must prepare a cold packed lunch for day programs or work;
  • Assists with participant’s behavioral challenges, including but not limited to physical restraint;
  • Assists with program participant’s medical issues, personal care and all aspects of hygiene/grooming, including but not limited to bathing, toileting, feeding, lifting, transferring, shaving, tooth brushing, medication assistance, etc.;
  • Lift, transfer non-ambulatory, following approved procedures;
  • Assists with AM transport as assigned. Provides transportation in agency vehicles to community activities, appointments, therapies, work sites, day programs, etc.
  • Walks program participants into their day program, work site, activity, etc. and you must walk them into the site make contact with the staff at the site for the participant’s health and safety. No exceptions;
  • Abides by agency policies and procedures regarding transportation of program participants and maintenance of agency vehicles, implementation of vehicle safety (seat belts) etc.
  • Responsible for cleaning facility, vehicle, community area and program participant up after incontinence, eating, illness and behavioral incidents;
Medication:
  • Completes assistance with medication delivery;
  • Documents assisting with medication delivery, including assisting with PRN’s and documenting their results;
  • Maintains secure storage of medications;
Records Maintenance / Documentation:
  • Documents daily including but not limited to progress notes, medication assisted with, task list, support strategies, GERs, data tracking and completes other daily documentation as required by Medicaid, the NM DOH and PSSI;
  • Required to document in all company assigned electronic documentation modules including but not limited to Itherapy, Therap, Quickmar, etc.
  • Completes any and all documentation during the overnight shift and during any other scheduled shift hours;
Home Maintenance:
  • Maintains cleanliness of residential home, yard and agency vehicle;
  • Assures cleanliness/tidiness of bedrooms and bathrooms of individuals supported, including assisting with making beds in the AM before transport and changing linens;
  • Performs daily cleaning duties in accordance with the house task list and other cleaning duties as assigned, assuring all items on the list are complete for the day before the end of the scheduled shift.
Miscellaneous:
  • Maintains confidentiality - has access to client Program and Medical records maintained in the facilities and electronically - abides by agency confidentiality policy and state, federal and HIPAA regulations;
  • Attends house meetings, DOH DDSD required trainings, therapy trainings, facility and other meetings as scheduled.
  • Keep personal calls to a minimum/acknowledge call waiting;
  • Laundry needs to be completed by the end of shift for the house and individuals supported. IE: folded/ hung up/ put away. Do not leave it in the washer or the dryer. Assure that all dirty laundry is in the clothes hamper prior to leaving;
  • Make sure all of the beds are made neatly before leaving for transport or leaving your shift;
  • Do not bring fast food, sodas, caffeine, or candy into the house. Never eat these type of items in front of the program participants;
  • Do not take the program participants to convenience stores or fast food places for sugary/caffeinated drinks, chips, greasy food, etc. No exceptions;
  • All cleaning on the overnight task list needs to be completed by the end of shift. The house needs to be left clean: IE: dishes done, floors swept and mopped, bathrooms clean, table and counters wiped off, meds and books locked up, all lights off, all doors locked;
  • Follow all speed limits and any vehicle/transport policies. IE: no smoking, eating, or drinking in the vehicles; no exceptions;
  • All program participants must be well groomed, well dressed and dressed according to weather conditions;
  • Awake Overnight Counselors are responsible for making sure that all of the program participants are sent to work/day program with a good well balanced cold packed lunch that follows their dietary guidelines;
  • Awake Overnight Counselors need to ensure that all program participants have a nutritious well balanced breakfast that follows their nutritional guidelines and menus;
  • Have all of the program participants to their day program/work site on time. Wake them up with ample time to assist them in getting bathed, dressed, meds, and eating breakfast;
  • All documentation must be completed prior to leaving shift, including but not limited to progress notes, sleep logs, medication records/MARS/counts;
  • Other duties as assigned/requested;
Crisis Intervention Technique:
  • Follow all behavior support plans for program participant;
  • Utilize the Company’s approved crisis intervention technique if approved in the behavior support plan;
  • Employees must a step back when a program participant has a challenging event and/or behavior and restraint is used as a last resort. Utilize restraint only if the program participant is trying to physically hurt themselves, someone else or put themselves in immediate danger.
Safety Sensitive Position:
The Awake Overnight Counselor position is a safety sensitive position where the employee holding this position has the responsibility for their own health and safety, as well as, the health and safety of the program participants, their co-workers, community members, team members and stakeholders. This position has many safety sensitive duties including but not limited to working directly with program participants, assisting with medication, preparing food, and operating a motor vehicle regularly for the company.

Working Conditions, Physical Exertion, and Dexterity Job Requirements:

  • Works primarily in a residential setting;
  • Required to stay awake during the overnight hours;
  • May be requested to work other shifts or facilities; Shift times, days, position transfers, and facility assignments may change and occur as necessary and without notice;
  • Must be able to physically perform all job duties. Moderate physical exertion required, including but not limited to bending, sitting, standing, squatting, kneeling, running, walking, climbing stairs, lifting, transferring, participating in sport activities, etc. Must be able to stand and walk continuously;
  • Must be able to lift 25 pounds above the shoulders, 40 lbs. or more repeatedly, and support 70 to 100 pounds;
  • Must be able to safely transfer program participants from chair to car/van, assist with toileting, and other duties related to transfer/ambulation; including rising from and sitting down and laying down positions. 
  • Ability to relate positively to the individuals supported and their teams;
  • Able to work directly with program participants including but not limited to challenging behaviors, physical restraint, medical care, personal care, grooming, hygiene, assisting with medication, lifting and transferring through the entirety of the shift;
  • Ability to work alone with program participants who are or can become verbally and/or physically aggressive on a daily basis and support them through crisis events;
  • Ability to hear and withstand repetitive noises, sounds, phrases, yelling, screaming, etc.;
  • Ability to assist, support, lift and transfer individuals who utilize walkers, wheelchairs and gait belts;
  • Able to perform the essential functions of the job with reasonable accommodation;
  • Able to work independently and without constant supervision;
  • Mandatory influenza vaccine annually;

Education, Training, Licensure and Other Job Requirements:

  • 21 years of age or older (required by company commercial insurance)
  • High School diploma/GED required
  • Must be able to type, have good computer skills and able to navigate the world wide web
  • Must have a clear driving record at hire and throughout the course of employment - with less than 2 moving violations within the last 2 years and no DUI/DWI’s within the last 5 years;
  • Must be able to pass a pre-hire drug/alcohol screen and continue to pass screens throughout the course of employment;
  • Must have a clear criminal background at hire and throughout the course of employment, including but not limited to COR/EAR check, NM Courts, sex offenders, receiving a clearance letter through DOH CCHSP, etc.;
  • Must have a current valid NM driver’s license at hire and throughout the course of employment;
  • Must have reliable transportation to and from work throughout the course of employment;
  • Must be able to operate company vehicles, drive for the company and transport program participants in a company vehicle throughout the course of employment;
  • Must comply with all training requirements, including but not limited to DOH state mandated and therapy training; 
  • Must be able to wear a mask covering your nose and mouth for the duration of your scheduled shifts; 
  • Must be COVID-19 vaccinated per State of NM mandate for healthcare workers;
  • Preferred but not required: have and maintain current personal liability auto insurance in your own name or provide proof your name is on the policy at all times while employed;
